When the developers throw one random QTE into the game just because it is the "in" thing to do. Seriously, if you must use QTEs then make them a major part of the game...Don't just throw one random "PRESS A OR DIE!!!!!!" moment halfway through the fucking game.

Having a time limit in a place that you have to escape from before it blows up. I get to the exit and litterally 1 second before I bet out, BOOM! Start whole thing over again.
