Probably the ending to the battle against the lava...armadillo...thing in Ninja Gaiden 2. After a long a grueling boss fight, if you don't hold down block, he explodes and kills you, forcing you to start the battle all over again.

Biggest WTF! moment was when you first 'fight' Pyramid Head in SH2. I tried everything; I tried shooting, punching, and even the classic 'kill it with fire!' philosophy, but he kept marching on. (And thinking to myself, "Oh god, he's gonna either kill or rape me! Or worse, BOTH!!!")
