Consoles shipped would include the number of consoles shipped to retail stores only, minus the ones that were returned from the retailers (returns, damaged shipments, etc). For Microsoft, or any manufacturer, these are what they consider their sales numbers and they keep track of them very closely. Actual sales to consumers are much harder to keep track of, since they have to get those numbers from individual retailers.
Consoles sent out from repair facilities do not count as 'sales' - they're exchanges covered under their warranty programs. They have to be kept track of separately from sales records for accounting purposes (sales are a profit, returns/repairs are a loss).
You should be able to take the numbers at face value, it was inevitable - the Xbox 360 is a much better console than the original Xbox, with a larger variety of better games. It would have been major news if it
hadn't sold better than the original.
