Braid. You won't believe the amount of times I've replayed that game. So... Damn... Gooood...
Other than that, STALKER: Shadow of Chernobyl and Kingdom Hearts 2 probably tie for the second place.

Dark Cloud 2 (Dark Chronicle in EU). Personally one of my favorite games of all time, perfectly presented, great music and gameplay mechanics. I've done so many replays of it and not once has it felt old. In fact I think I might go play some golf, or fish in game for a while.
Ratchet & Clank 2 (PS2) 20 times.
Jak 3 (PS2) about 10 times.

Uncharted 2 (PS3) 4 times.

Megaman Battle Network 3, finished at least 4 times. Same with Pokemon games, all of them - Silver/Gold and Blue/Red generation.
Mass Effect 1, twice, Mass Effect 2, three times and now fourth mid-way on Insanity.
Diablo 2, countless times, can't even count it.
Shin Megami Tensei: Devil Survivor about 2 times on the DS, for 2 different endings, now doing a 3rd one for the Neutral ending (already done the Bad Non-Evil ending and the Good ending), then maybe 4th time for the Bad Evil Ending.
I think that it also might count Persona 3 (70 hours spent) and Persona 4 (75 hours spent), and going to replay P4 at least once more.
When it comes to hours played, Diablo 2 has hundreds of hours, same with Neverwinter Nights. TF2 also 140 hours in total.

KOTOR/KOTOR2, Morrowind/Oblivion, Neverwinter Nights, Pokemon(over several generations but its basically replaying the same game whatever title), Fable Lost Chapters are all rpgs i have come back to again and again and still been sucked into their worlds.
I generally find non rpgs have limited replay value but as a young teen i replayed rayman 2 the great escape, halo and jedi knight jedi academy ridiculous amounts,less due to their replay value and more due to my lack of other games. Having said that with the possible exception of jedi academy theyre great games and jedi academy is still fun if not fairly lame.

Morrowind, most definitely. If I actually totalled up the time I've spent playing that game I'd have a heart attack. And since I still haven't done the quests for the three different vampire clans I'll be playing it for some time to come.
Diablo 2 as well - I never get bored of that. And Fallout 2 come to think of it.

X-Com: UFO Defense, Lunar: The Silver Star, both the original Sega CD game and the PlayStation remake
FFVII atleast ten times if not more. Guild Wars I have gone through every campaign atleast 4 times. Oblivion and the MGS 1-3(PS1 ones not the actual 1 to 3).
