Donkey Kong Country 2. Been playing it since '95.
Super Smash Bros Melee I played from 2001 until 2008ish. Constantly.

I've also played through FFVII at least 8 or 10 times.

Oh, and StarCraft. I clocked a lot of hours into that game. At least two whole summers that I can remember.

I've played the Baldur's Gate series more times than I can count. Its just that good.

Chrono Trigger/FFVI/Earthbound are up there. I've beaten each one at least 35 times for all the different endings/speed games/challenges I set myself to.

I've beaten FF6 more times than I can count with every party configuration you can imagine. Overleveled, underleveled, speedrun, savor every delicious morsel, you name it.

Probably Spider-Man 2. I've probably beaten the story missions around a dozen times by now, and then of course there's the time I've spent just free-roaming. It's probably hundreds upon hundreds of hours.

Well I have hundreds of hours logged in Super Smash Bros. Melee, if that counts.

Beyond that, I'd say Halo: CE and Prince of Persia: The Sands of Time are the two most. Halo I played a lot because it's a great freaking game, and I played it a lot with my cousin: the co-op campaign is a lot more fun than one-on-one multiplayer battles. Then I actually got my own Xbox and played it through multiple times after that. Sands of Time was just an amazing game, still one of my all-time favorites, and... well, in short... the first time I played it, it took me about 10 hours. By the last time I played it, it took me 4 and a half. I recently went back to replay it, and I still basically have the thing memorized.

I'd say a tie between Majora's Mask, Metal Gear Solid 2 and Kingdom Hearts 2.

All of which I believe I've played at least 8 times each.

Majora's Mask is my ultimate replay game. The slowly descending moon still creeps me out to this day.

Also If i EVER have a spare hour I'll always go for a quick blast through of Mega Man II

Without a doubt #1 is Starcraft and #2 is X-Com. MOO2, the Civ games, and Company of Heroes would round out the top 5.
