Mine, for instance, happened when I was DMing in D&D. It went something like this:
The situation was that the players were at a royal ball. My friend (Let's go by his character name) Rowland rolled a 20 and started playing a game of beer pong with the king and his court. They got utterly hammered and James (a rogue) was attempting to steal things (but he couldn't touch any metal for a month, due to another quest. Long story). He got on the roof and, to evade guards (who didn't care he was there anyway), he jumped off the roof through a skylight. The party stopped immediately when he hit the ground, but Rowland broke the silence by saying a racial joke. He rolled a 20. People loved it. James broke nearly all of his bones and struggled to stay alive while everyone else partied on. The entire court considered it "epic."
