Because of a final boss spamming an unblockable, near impossible to dodge one hit kill bullshit attack.
The last time I threw a controller was when playing the new Alone in the Dark. It was the level where you have to drive through a city being destroyed. If you've played it, you'll know what I'm talking about. After falling through the highway for the 15th time, I threw my 360 controller, but made sure I threw it down on the couch. Those things are damn expensive. After stating several expletives, I calmed down, and got it after two more tries. I still have fond memories of chucking my controller at my brother when he spammed E. Honda's hundred hand slap in Street Fighter 2.
Usually I don't get mad enough to throw my controller, and before the Alone in the Dark episode, I think the last time I threw my controller was 1993. Usually I just stop playing, but that damn game just pissed me off enough. Mainly because I knew I could beat the level.

It's probably on the floor because I tossed it from my couch to my TV, where I keep it when I'm not using it.
Nope, no ragequitting and whatnot from me. Although I think I've made some other people ragequit. I know that when my friend and I played 1v1 in Bioshock 2 multiplayer, I beat him in four games in a row by a good amount and he refused to talk to me for the rest of that day. I explained that he never changes his strategy, but he never learned that adapting is what lets you live. XD

I've never been a controller destroyer, or thrower for that matter. I do however give my controllers the occasional soft lob onto the couch, generally because I'm getting out controllers, or because I'm done for now, and too lazy to put it away properly at that time- I tend to force myself to stop before I get to that point where it's time to projectilize my input devices. But if I ever get to that point, it will probably be but one reason: pure unadulterated frustration.

Because it's impossible! No one can do this shit! The fuck are you supposed to beat this!? He's too fast! There's not enough time! Goddamn he's overpowered! A fucking flamethrower though!? Where the fuck is his weak point!? There's no way that hit me! I fucking dodged that! I already know, I've seen this cutscene a thousand times now! Come on, re-fucking-start already!
