Red Dead Redemption. Unpaused game, blindsided by cougar.

Lucky you, I'd love to be blindsided by a Cougar when I play that game, all that happens to me is that I get mauled by mountain lions.

OT: AFKed a lot in WoW and died quite a few times as a result, its just one of those inevitable things.

AFK'd Minecraft accidently at the spawn, came back to find a very large hole in the earth.

AFK'd on Deadspace 2 for 15 minutes AND NOTHING HAPPEND! then i made Isaac step forward and 2 necro's jumped me. scripted/triggered enemy encounters suck balls! I want my enemies to be freerange so they can jump me ANYTIME and ANYWHERE!

AFK'd on CoD (360) many times, come back after taking a piss to find I've been kicked for inactivity. Very infuriating.

AFK'd during a raid on Warcraft, got kicked from the guild.

Also whilst playing Red Dead Redemption, we were playing free-mode holding a fort thingy, with cannons shooting other players, and I had to afk. So I ran into a corner and went, came back to find someone had kited a few cougars back to the fort and annihilated everyone.

Went afk and hide against a wall. Came back three minutes later, entire team down, enemies all standing at my spawn waiting, thinking I was going to rescue the hostages.

Funniest TD in a while.

I forgot to quit a game of Black Ops once while chatting to the missus. Came back and was 0-15.
