Subject says most of it.
For me the best plot twists happened in two games.
Baldur's gate 1: When I found out that Seravok is actually Kovares spelled backwards, And that I spoke to him that whole time.
Baldur's gate 2: When I found out Imoen was a bhaalspawn aswell, I had no idea. :(

Worst plot twist:
Neverwinter nights 1: When Lady Aribeth turns evil, I mean, That was so expected, At the first episode when you find clues about that Cult, When you get the idea that the cult has a symbol of an eye, And then at the end of the first chapter, You see a cinematic with Lady Aribeth holding a sword that's on it's hilt was the symbol of the cult.

Feel free to post your own Best/Worst plot twists
Worse than Aribeth was Desther. He could not have been more obviously an evil traitor-spy if he had been wearing a Pavel-skin coat.
You: I've found out lots of really intriguing stuff, and killed lots of bad guys and rescued half the city from certain death.
Desther: You IDIOT! While you've been WASTING time doing that, I've made this lovely baby stew. Fenthick! Arrest him immediately!
Fenthick: Gee Desther, I'm just not sure about that. Can't we all just get along?

THE worst:

Metal Gear Solid 2
The biggest twist for me was realising that they actually did expect you to play as Raiden all the way through, rather than it being some tortuous intro bit.

Baldur's Gate 2: Yoshimo.

That was the best twist because I lost a party member that was funny, likeable, and useful as hell. It also helps that I did not see it coming from a bloody mile away.
Ehh actually my brother quit the game after that bit, because he was basically the only character he liked and I don't think you can get him back again.

A rather interesting one was Dead Space...
with Nicole being dead the whole time, that was kinda obvious. I did however like how the first letter of all 12 missions spelled out NICOLE IS DEAD
New Arrivals
Intensive Care
Course Correction
Obliteration Imminent
Lethal Devotion
Environmental Hazard

Into the Void
Search and Rescue

Dead on Arrival
End of Days
Alternate Solutions
Dead Space .]
Holy shit! I never noticed that!
Holy crap that is amazing. That game just got a lot better for me.

Best:

Silent Hill 2
James killed his wife and it just happened like a week before the game. Up until that point James seemed like a really sympathetic character, so the scene really leaves you with a bad taste in your mouth and your heart in your stomach.

One of the worst:

Final Fantasy X
Talk about cliched "it was all a dream" plot twists. FFX's twist was "It was all a dream. The world you knew was a dream. Your friends and family were a dream. You're a dream. You'll cease to exist when we wake up. Say goodbye to your happy ending. Screw you. Love, the Faith."

THE worst:

Metal Gear Solid 2
Ah ha! The villain behind it all was Raiden's pseudo father Solidus (another clone of Big Boss) that he doesn't remember at all! The Colonel is an A.I.! Rose might be an A.I. (but who cares?) The whole Big Shell incident was a training operation created by Ocelot for the purpose of creating a new Solid Snake! Solidus killed Raiden's real parents! Liquid has taken control of Ocelot through his arm! Liquid/Ocelot steals Metal Gear Ray to do....something! The A.I./Colonel is one of the Patriots! Rose is real and not an A.I.! The Big Shell incident wasn't really being used to create a new Solid Snake, it was to test some program to...control humanity through...something or other.

I don't know the rest, that was around the time that my brain shut itself down out of self-preservation. Let that be a lesson, too many plot twists can turn a story into a piece of incoherent garbage.
I actually really liked the ending of Metal Gear Solid 2. It might have been over-the-top, ridiculous and pretty far fetched, but for some reason I like it when storylines manage to completely fool me. The whole arm thing was pretty stupid though and I'm glad they retconned it in MGS4.

The worst is in every single game where you get taken out by some unfortunate situation. that given the freedom to play it out yourself you would of avoided by either not being a complete mug or just being more skillful than the game gives your character credit for. And then you wake up with out your weapons and Items and have to start again. Usually swearing vengeance.

I know everybody was disappointed by Assassin's Creed's actual ending, but...
Even though the ending was a blatant opening for a sequel and anybody could have seen Al Mualim's betrayal coming (my brother knew that "my boss" was going to be the final bad guy from the beginning), I still liked the way the final missions went. What I mean is, you don't expect any bright lights or weird, futuristic stuff in a Medieval game, right? Well, I was weirded-out when I came back to Masyaf and I saw that everybody had gone crazy, and I was even more surprised when I walked into the castle courtyard and suddenly, Altaïr was glowing yellow and paralyzed. Then the part where all the guys I've killed come back, and Al Mualim multiplies himself a dozen times, plus the hologram at the end with the alien-ish music... it was all icing on the cake. You gotta at least give them credit for THAT much.

Another worst, Morrowind: Tribunal
Who didn't think that the Goddess would end up betraying you? All the bull she'd been doing recently should have been an alarm

And now a word *I mean worst* from Oblivion.
Two instances of this. One in the Elven Gardens district in which a man says that a girl he was having an affair with was killed by a vampire. He sends you out to a hut to kill the guy, who then tells the truth and sends you back, where you find the quest giver waiting in full armor to kill you. Also, in Bruma I believe, a man is killed by a "vampire hunter" as is another in Cheydinhall. Turns out the three of them were all old buddies with a treasure. Need I go on?
I don't remember any good plot twists in either of those games at all.

Viewtiful Joe plot twists anyone?
So corny yet so...well corny.

Worst was probably Twilight Princess
You had to fight Ganon so the original bad guy was just a puppet

Well, KOTOR has been said, so...

People have said FF6 had a good plot twist.

WHERE IS THERE A TWIST IN FF6???
>Terra is Half-Esper
>Shadow is Relm's Father
>Sabin gave up claim to the crown to study martial arts so he can one day aid Edgar
>Locke was engaged, and searches for the Phoenix Magicite to save his mummified betrothed
>Celes is with the good guys, no she's not, oh wait, she really is, or is she?
>Mog can talk
>Gau's father is the crazy guy in the cabin
>Cyan is possessed by a demon
>Strago's friend wasn't really wounded by Hidon
>Setzer didn't own the world's only airship. His friend had a better, faster one, but died.

And as for my own contribution:

When Sora stabs himself in the chest in Kingdom Hearts in order to free his friend. Freaked me the hell out because he was smiling the whole time.

The entire ending of MGS4. I mean, seriously, what the fuck? If that's supposed to be good after playing MGS one through three, then they must cause severe brain damage.

Good, KotOR, obviously, and finding the flood in Halo: CE was a wee bit expected, but the cutscene after the Library, wherein you find that Halo kills everything in the galaxy and finding out that the lightbulb wants to kill you, that I loved very much.

Outside of gaming, I'd have to mention the ending to book Fight Club. Better ending than the movie, and far more spine chilling.

I won't go as far as best, but a very satisfying one was in the original MGS when
the computer says "Detonation code activated", right after you insert the 3rd Card Key. I sooooo did not expect that I'd actually fight REX.

I also loved ever twist of MGS2, possibly with the exception of
Rose being a spy
, because you'd have to be blind not to see that coming from a few planets away. I'd put it on top of the list, but the whole game breathes on twists and I'm biased.

The "twist" in Silent Hill Homecoming was pretty underwhelming, since pretty much everyone seemed to have figured it out, except Alex and the story took too long to get to a revelation everyone knew about already. "Portable Ops" had a few nauseating twists, especially
Sokolov being alive and Operation Snake Eater being orchestrated.

Where you find that you aren't having flashbacks, but the flashbacks are actually the present and you're having a twisted vision of the future.
excellent game, I still have it, its the only game in the world that can never have a sequel.
And I don't mean because Free Radical aren't in business, but because the future in which the game happens can only come to fruition providing the bad guy is alive, and you kick his arse 7 ways from Sunday

Really lame plot twists for FF3

Oh, you all just happen to be the chosen ones. Oh yeah, and you're brother and sister.

Good plot twist for FF3

But hey, what you thought was the world is a a very small continent, and there's at least 3x the game left for you to explore!

Good: I'm going to have to say Legacy of Kain: Soul Reaver 2. Seriously, did anybody see that coming? Time travel = delicious mind-fuck.
I fully agree, there aren't better plot twists than those in that game.

As for bad. I would say the one in dark messiah
Uhhh you are the Dark Messiah, who the hell was expecting that? Uhhh Xana is a demon, no one was expecting that either. But I still love her.

Holy shit! I never noticed that!
My exact reaction right there...enlightening.

Megaman 5.

Now keep in mind, this is NES, and plotlines of megaman are as follows: Wily goes batshit bonkers, makes 8 master robots to take over the world. Megaman is sent to keep the idiot in check.

Megaman 5 starts out differently. Dr. Light has been kidnapped, and the napper is....PROTOMAN?! And of course he has 8 master robots that plan to take over the world....etc.

After beating all 8 master robots and fighting through 4 more grueling levels in Protoman's fortress, you comfront him. Only to realise it's an imposter! The real Protoman shows up and give you something to defeat the imposter Darkman. when you think that its all over, it isn't. That bastard Wily shows up and reveals he was behind the entire thing! And then you go to his fortress, 4 even more grueling levels later you defeat him and its finally done. Megaman saves the day!

My god, back in those days...That many plot twists boggled the mind! Capcom writing at its finest!
damn straight its Capcoms writing at its finest, what about when Megaman almost gets crushed and Dr Light quips about him "almost being a mega sandwich"
