The most annoying one was in STALKER: Clear Sky, I couldn't stop bleeding, no matter what I did.

True crime: streets of L.A. where anytime you walk onto a slope the ground would become invisible and a magical upside-down world was revealed. Incidentally the game was very cheap and I later sold it, thus making it pretty much free. That's my excuse for owning such a piece of crap and I'm sticking to it.

the Crucible glitch on Fable 2.

complete the crucible with 8/8 perfect rounds, then go back and do it again. if you get another 8/8, the super-prize chest becomes available agin.
opening the chest teleports you out to the front of the crucible (as it should) but because you already have the prize, the thinks you're still trying to get the reward from the chest, which isn't there.

you end up frozen, bent over slightly, unable to do ANYTHING at all.

the patch that came out (eventually) resolved this issue. the L/H community also discovered a workaround to unfreeze yourself

Hylide. Where i'd turn the game on and just walk around in a garden with no way to exit. Medevil gardening simulator FTW

STALKER: Almost the entire game. I still love it to bits, but the most bug-ridden masterpiece ever.

FC2: Constant 'reboot my PC' bug every few hours or so and unalterable window mode (although I found a file edit solution to this later).

I always had that trouble with the Nuka-Lurks they never didn't do that.

Another really fun one that I'm surprised no one has mentioned was GTA4's "Swing-Shot". There was a swing set that would sen your car flying hundreds of feet in the air if you parked under it.
Yah, but who parks their cars under a swing set anyway?

Gears 2, quite near the end you need both players to open a door by turning the big wheel things.
My friend was waiting for me, and i had a boomershield, i stuck it in the ground for safe keeping while i opened the door.
We complete the whole turning thing and it cuts back to the normal play screen. Yet the gate hasn't opened, and then i noticed the shield wedged into the gate itself, holding it shut.
Thank god it was right after a check point!

Neverwinter Nights 2, before they finally patched it up, there was a game stopping bug about 1/2 to 3/4 of the way through the game. And of course, I came across it and I had to start all the way over again (which I didn't bother doing for a long time afterward.)

Any glitch that forces you to load a save... especially if you don't have one.

Saint's row 2. play the whole game, love it, buy EVERYTHING, get to the last mission.. bug.. The last mission never shows itself. I cannot start it. I can just run around the area it is supposed to be.

I almost cried.

I've had many bugs and glitches in my gaming experiences, L4D, Fallout 3, GTA IV and Oblivion.
I've played many other games but hell I don't remember bugs from them!
Anyways, Fallout 3 had the most bugs I have ever seen in a game! I mean seriously oh my lord!
Every time I want to enter Megaton (The first city) it crashes.
But wait when I do get in it crashes every time I step outside a house after entering it!

How does one get underground? I've been everywhere. In the caves, in the sewers and the access to those was halfway across the map. there is not access to underground in the mission location.

Go to the last mission location, go inside where you took out the limo, head to the elevators like before, go down, the mission is in the mall.

I think the worst (as in "who could miss this in QA??") is the old W-Item trick in Final Fantasy VII that basically allowed you to have infinite items.

It's really a big bug when you consider the fact that it was back in the ROM days when companies weren't able to patch a game.

But it is worth noting it is a good bug.
