OT: Before now I was being a dick and decided it'd be fun to abandon my helicopter over the middle of the city. I dived out, not really expecting to live, hit a skyscraper and bounced back off, falling to my death. My friend, on the other hand, held tight for a few moments, dived out, and proceeded to fall a VERY long way into a nearby duck pond. And survived.

ok this is more of a worst fail for me.
i was playing San Andres and i had been trying to complete a really annoying mission for like over an hour. I had to steal some plans from a office building. It takes about 10 min of hoop jumping before you make it to the top where the plans are kept after which the alarm goes off and you have to kill tons of cops to escape. If you die then you have to do it all over again, so after half a dozen tries I fianlly manage to fight my way out of the building and i manage to shake my wanted level and since i was right near a gun shop and on my last leg, i decide to buy some new armor just in case any last minute surprises we're waiting for me. I buy the armor, leave the store and get in my car...and then i shit you not, a goddamn PLANE!!! crashes right on top of me, blowing my car up, killing me instantly and forcing me to restart the mission over again!
I know what happend, when leaving a store the game respawns people, cars and other stuff that it removes while your inside a building to save memory. one of the things that was spawned was a plane, but it was spawned right in front of a high rise building causing it to insantly crash and kill me...but what are the odds that it would happen then of all times!
I had to rage quit (one of the few times in my life) and i didn't touch the game for a few days. It is the cheapest death i have ever gotten in a game in my life and it's one of the many reasons i will proabley never play that game again (another was i just didn't like how it ended)

Getting shot by a cop in GTA4, a cop who apparently only took a brief and rejuvenating nap after having been shot in the face three times and taken for dead by me. GTA4 really had some issues...
In IV, getting chased by cops whilst on an NRG, riding off towards the airport to steal a helicopter. Driving along the highway, not paying attention to much except shooting the cops chasing me, I turn around just in time to see I'm approaching a billboard. Not a problem thinks I, as I hit the brakes... as I hit the median strip and thus lose contact with the road, landing just in front of the the billboard with all my forward momentum still in place. I'm very surprised Niko didn't lose his head.
I was flying a jet around the forbidden area's I hadn't unlocked yet and had military jets flying all around me. One of them collides headlong into the side of the jet I'm flying. I bail out in a fit of panic. About a minute later I pull the cord on my parachute.
Somehow, amazingly, the plane i was flying didn't catch fire and explode as I thought it would. When it got hit in the side, it canceled out all it's forward movement somehow and began falling straight down. About fifteen seconds after I pulled the cord the plane I was flying falls right on top of me while I'm still about 25,000 feet in the air.

There was a time i was on a 4 star wanted level in IV riding away on a cycle and just about to get away from them and then the turn before i could get out of range i braked entirely to early and missed the turn went smack dab into a short wall and went flying off my bike human cannonball style several yards from where i crashed and then into the side of the building dying instantly and i was left there saying "damn that had to hurt" in between my bouts of laughter.
I would say playing GTA 4 all the way through even though I was terribly bored the entire time.

Me and a few friends were in a helicopter, the blades broke when we crashed into a sky scraper, me and my friends both started screaming "WAAAAAAAAAAAAAAAAHOOOOOOOOOOOOOOOO!!!" and landed on some poor poor guy.
A fail that happened to me:
One guy was speeding towards me while I was on foot, so I jumped out of the way...
and into another fucking car which knocked me down and gave the other car time to turn around and run my ass over.

OP: well if I can ignore GTA4, I'd have to say one time in San Andreas, I was in a huge shoot out with a bunch of cops and I was almost dead when I finally finished them all off BUT OUT OF NOWHERE one comes up behind me with a shotgun and kills me. I was like "HE JUST WENT SCARFACE ALL OVER MY ASS!"
I took Little Jacob to see a show, when I dropped him back at the cafe he punched me in the jaw, kicked me out of the car and drove off. His door wasn't even blocked.
