According to Swedish tradition (country of origin), the family bakes a cake the night before (always chocolate in my family) and wakes up the birthday person with cake, candles, presents, and lots of hugs n kisses n photos (moms!).
Other than that, we usually let the person become sort of a "king for d day" with first picks for anything: choice of movie, first on turn to play game (or exclusiveness if they wish), decide what's for lunch and dinner, etc.
Admittedly, this will get harder and harder as the kids are all adults now and we are severely separated (seriously, five people across three continents). But my sister's friends learned of the cake waking tradition, and decided to do it for her, since she's in uni away from the rest of us (hopefully we will all find such nice friends).
