From my experience, all the paths are actually quite logical in their outcome, but if one is playing through it the first time, then obviously we wouldn't have any knowledge of the outcome. For example, in the case of Shizune vs Lilly, of course siding with one girl will cause her to think 'less' of you, and it reflects on Hisao himself that the "personality" you choose for him is what lead him to that choice. if you want to go with Hanako then DONT put in extra effort when you go running for the first time with emi...I think thsi will lead you to lilly...go to the libary when it becomes an option and you should be good (and be nice to lily)
On Emi's path, it's actually quite "logical" for Hisao to take it easy considering his heart condition, and you'd only really choose the "run harder" option if you're aiming for Emi's route.
It basically works that all the choices you make in act are are shaping Hisao into the kind of person that will like a particular girl in question, which means the more interested or favorable he is to a girl or spend more time with them, then obviously that's who they'll want to stick with.
That said, yes there are choices that seem to be put there just for the hell of it, like one choice regarding Kenji in Lilly's path, where choosing either leads to the same thing and doesn't affect the story as a whole.
