I've successfully pulled off a couple of no-death runs, but I don't have the discipline to do stuff like drop ammo or keep track of time to eat and sleep.

I'm looking forward to New Vegas and its Hardcore Mode, which will do by coding what I fail to do by discipline.

I don't know about the overall game, but I did what I call a "Scav Run" of Point Lookout.

That is to say, I left all my belongings (ALL of them) including ammo, caps, clothes, etc. in my house in Megaton, walked to the ferry for Point Lookout (beat a Mirelurk in hand-to-hand combat, but luckily ran into nothing else on the way).

Once I got to Point Lookout, I only used what I could find. Find one grenade on the ground? Make it count. Find a leather coat? Yay! DR higher than 0! Find a 10mm pistol? Pray hard for some 10mm ammo to show up, too.

It was pretty difficult at first, but Point Lookout has more than enough equipment lying around for you to build up a decent arsenal. Still... no specialty weapons, and it's always a scramble to find enough ammo and health packs.

I would say these.
Get addicted to a chem and never get it cured.
No max level perks or things like Animal Friend or buffs against other enemies.
No Stimpaks in combat.
Only repair guns once, or just no repairing guns.
No special weapons.
No high defense armors.
No Stealing.

That is true, along with unarmed too.
Maybe limit himself to a Baseball Bat or a Knife and his Bare Fists too, to combat that.

Oh, to make it a little harder, no Unique Weapons either. The Xualong [sp?] Assault Rifle is too awesome to use.

So you set an ammo and carry limit at a level which is way above the needed amount, and labeled it hardcore?

Please you don't even have a single line about stimpacks. This isn't hardcore at all.

Even with ten shots, you pretty much can kill ten of anything that isn't super massive with a well placed headshot.

Then may I humbly suggest that you offer some constructive criticism, instead of just sniping at the OP for his supposed lack of e-cojones?

OT: A couple suggestions.

-You may not use traders to repair weapons or armor. All repairs must be performed in the field.
-You may not use stimpaks to heal crippled limbs. (Or if you insist on it, limit it to healing concussions--which is actually pretty reasonable, seeing as how the concussion effects are some of the most annoying in the game.)
-You may ONLY use one kind of weapon. Small Guns, Melee, whatever it may be...I hope you're a fan of it, because it's the only thing you'll be seeing throughout your playthrough.
-You may not use Power Armor.
-You may not take any perk that replenishes or increases DR, Rad Resistance, AP, or chance to hit in VATS.

Download Fallout wanderer's edition. It puts a weight on ammo, and a bunch of other stuff that re balances the game to make it harder without stupid little restrictions.

- No quests from anyone.
- Infact roleplay as a lone raider, no friends or talking of any kind unless it's vital for the main quest.
- Murder everyone in all the towns except Underworld and Rivet City with as little buildup as possible, at least equipment wise.

Try Hobo-Hardcore:

Melee weapons only.
Must become addicted to all chems and alcohol as soon as possible.
Never have your own place (so don't complete the Megaton bomb quest, either way).

Very much this. If you're attempting to make the game "harder" or "more realistic", this is the way to go.

Good on you OP for trying a different playthrough. I just finished a Pacifist FO3 playthrough which was a lot of fun.

Download Mart's Mutant Mod and add in a bunch of bigger, scarier monsters. Night Ghouls, Wananimingos (lolspelling) and Deathclaw Matriachs can be kinda scary, especially if you used the Increased Spawns module and Feral Ghoul Rampage mods.

Thanks for all the rules everyone, I've decided on my final ruleset and it's as follows;
-I Can save (In case the game freezes)
-When I'm dead, I'm dead for good
-I can only carry enough ammo for 15 magazines of each type of gun I currently have (E.G. if I'm carrying a 10 mm pistol that gets 10 rounds a magazine, I can only carry 150 10 mmbullets at one point)
-I can only carry three guns at all times, minus ones I use for repairs
-I cannot use any followers
-Must discover all locations in the base map (None from any of the dlcs)
-I cannot exceed 200 pounds of combined weight
-All bobble heads must be collected
-Cannot use Ghoul Mask in any metro
-May only use Ghoul Mask if I decided to go with the option of letting the ghouls in and having them slaughter everything
-All energy weapons and .44 magnum ammo must be looted, the rest can be bought.
-No using anything that increases my AP
-No fast traveling
-No using the Megaton or Tenpenny house
-Only 3 stimpaks can be carried at all times
-No explosives besides the Miss Launcher
-Melee weapons can only be my Fists, Baseball bat, and pool cue
-Can only wear my Vault 101 jumpsuit unless absolutely necessary (I.E. if I do end up play Operation Anchorage)
-Shotguns are off limits
-I cannot use any chems ever
-I can't take any perks that increase DR, or Rad Resistance
-Cannot do the Wasteland Survival Guide
-Megaton, and Tenpenny tower must be 100% cleared of all life
-I can only drink Dirty Water
-Fat man is off limits
-Xuan is off limits
-Miniguns, and Gatling Lasers can not be used
-Stimpaks can only be used to heal concussions
-Rad-X, and Rad Away are off limits
-All other injuries including radiation must be cured by a doctor
-Everything must be dropped after Vault 101 besides my jumpsuit..
-All repairs must be made by me
-Not allowed to use Grim Reapers Sprint, Explored, or any other perk that replenishes, increases, any thing beyond skill points.
-Deathclaw sanctuary must be completed before I go into vault 87..
-Can't use Animal Friend
-No using Intense Training, All SPECIAL stats can only be raised by their respective bobble head
-May only use the standard energy pistol
-If my energy pistol runs out of ammo, I cannot buy or pick up any more, I must sell it or drop it.
-All Behemoths must be killed back to back (With the exception of the Behemoth in the GNR quest)
-If a weapon breaks during a fight, and I have no way to repair it right away it must be dropped
-No traveling at night

I feel that sounds pretty good right there, also I'm playing on the Xbox360 version so I'm afraid I'm going to have to micromanage my ammo. Which wont' be too much of a pain, by only being allowed to carry three weapons (four including my fists) at any given time..

Oh and I can only use the standard energy pistol.

I was thinking about doing something like this for RE4 but then I could't be bothered to keep inventory of my ammo so it feel through. Good ideas with your rules though.
