Either dragon valor or chrono trigger would be high on the list.
The first medievil wins hands down though, for the game intro to the menu and the opening cinematic.
Nothing like seeing a sorcerer awakening the dead, and witnessing your own death again before given a second chance at life.
And, in contrast... X-Men 2: Clone Wars had the most unsettling intro: a cold open. You started playing as soon as you turned on the console. Not even a title screen until you beat the first level!
I have to agree on Homeworld and Fallout 2, but my favourite will forever be X - Beyond the Frontier, simply for how powerful this game was, going from usually only watching my father play the games and having an ancient greenscreen only game console to playing a real, full-sized game with the scale and depth of a 4X space simulation like X, it was sheer magnificence.
