Ninja Gaiden on the orignal xbox. I struggled with the first boss on the normal difficulty. I was younger though.
Same here, Though I would like to dust off my xbox and give it another try someday.

For me, the original Fallout. I've all but given up on it because I'm unable to finish the last part, not through lack of skill but because I'm not a high enough level, and lacking the gear. However, good equiment is extremely rare, I can't pick up laser rifles for some reason, and there's not enough content in the game for me to level up sufficiently.
Fallout 1 is insanely tough, because there are very few opportunities to grind and you can royally screw yourself by no fault of your own.

I'm sad to admit I did have to eventually resort to a walkthrough to finally finish it.

There was so much trial and error that it wasn't a challenge, just more like psychological torture.

Tales of Vesperia had this a few times where you're cruising along, then the boss comes and beats the ever loving shit out of you.
I beat this with a couple of good friends of mine and we still joke about how much fun the game was... until a boss would show up and wreck us multiple times in a row. I swear I thought I was too low level or something, but the bosses really just make you get your crap together or go home.

CoD: WaW on the hardest difficulty. The trench level was so evil... not to mention that the enemies respawned and were all took classes in how to F*** you over with grenades.

Kane and Lynch, the construction truck battle, beat it once and stopped playing then decided to play from the beginning and couldn't beat it again so gave up and played something else. I gave you 2 chances game you should only require 1.

every Halo game on Legendary is hard, but after a few hours at the most i usually get through a jam. Halo 2 was just pure ungodly ridiculous on legendary with the effing jackal snipers. completely unrealistic.
Lost Planet on Extreme difficulty is effing ludacris, the last boss on normal took a few hours to get halfway down. sheesh.

Psychonauts...ok before you say i dont know how to play video games, hear me out. In the beginning of Psychonauts it was so easy that i could beat it with one hand...you reach the last level...
Don't worry, The "Meat Circus" is widely known as being quite nightmarish thanks to its suddenly steep difficulty and...well, for being so twisted and bizarre.

Going beyond a single game and looking at a franchise, Rainbow Six Vegas 2 is SO. MUCH. HARDER. than the first game. And while a BIT more of a challenge was certainly welcome, it just was to frustrating for me halfway through. True, being punishingly realistic when it comes to being shot is closer to the roots of the Rainbow Six Franchise, but the real fans of Games like Raven Shield probably won't like the "Vegas" Games one way or another since they are far more streamlined and dumbed down in it's mechanics.

Also, any of the "GTA III" Games (III, Vice City and San Andreas) were fair enough for the most part, but all of them got pretty ridiculously hard towards the end.

And i never finished the "officialy" last of the sands of time games, Prince of Persia: The Two Thrones, because of some stupid bossfight against some sort of demon twins.

Modnation racers comes to mind, for a casual game it suddenly gets incredibly bitchy about 3/4 of the way through which makes it exceptionally annoying to play at some points

Mod Nation Racers SP campaign. The AI is ball-bustingly hard to race against and some of the tracks are so hard to maneuver in - the one I'm currently stuck on has a really tight curve around a lighthouse that has no railing. Its so hard in fact that if it weren't for the fact that you unlock things to use on mods and karts for completing objectives in the SP races, I'd stop trying forever.

I STILL haven't finished Brutal Legend. I feel like I've let Jack Black down, for no good reason. I'm sorry, all things awesome about that game. I just can't play it again, the last mission is insane, can't make units fast enough, then I lose the towers, and cannot make more units... Ahhh.

Viewtiful Joe, when they started pulling this "5 bosses in a row without a save point" business.

I'd argue that I Wanna Be The Guy isn't an example of true hardness so much as sadistic design. Running through a gauntlet of dodging fallen apples, only to have the last apple fall up and kill you is a dick move.

GRENADA GREN GREN GREN ADA GRGRGGRGRGRGGRRAANNANA DADADADADA. "Clink clink clink clink clink"
I know, i cant do the asylum level having proceeded thus far on luck and random sprinting between cover. How many grenades can one person have? And they cook them to boot!

Have you played any games that just got so hard they weren't fun to play anymore? JAK 2 comes to mind, I played it when I was still pretty new to gaming and that might be the reason I found it ridiculously hard.
Seriusly, am I the only one (I know that I'm not, shut up) who didn't find Jak 2 to be that hard?! I was like 10 when I beat it.

OT: Tekken 5.

Me:"Hey Tekken! I remember enjoying Tekken 3 on the Original Platstaion, maybe this game is even better."

Me after:"Oh god someone help me! I'm beeing spoon feed with my own testicles here! Oh god the pain!"

I stop sometime during most games when I have to restart something more than once. It gets boring if you dont do it on your first or at least second try.
I usually start up again after a week or so though.
I've never finished psychonauts though. The meat circus was annoying me.

Yes Jak 2 was the game that sprung to mind from just seeing the title of the topic.

I still found it fun and in some of my more masochistic moods I'll pick it up and try with tears to overcome the mass of evil that is some of those missions.

Ninja Gaiden deserves it's obligatory mention.

I do want to play Demons Souls just to join the legion of whiners but I'll withhold that for a rainy day.
