All time favourite is split between Persona 4's Reach Out to the Truth or Trials and Tribulations' Cornered theme.
Hmm, maybe I should go back and actually finish watching the Persona 4 anime...
Of course the original Cornered theme will forever be one of the greats.

It's so hard to pick, honestly. Music is so amazing and I'm endlessly overjoyed that (some) game companies haven't forgotten how integral music should be to an overall gaming experience.
Back when I was a kid, even with its (let's face it) really poor sound processor, the Genesis (Megadrive for people in the UK? Is that right? Sega's system, anyway) was home to the Sonic games, which had some of the most awesome videogame music ever.

In moderns games, though, I am often surprised and delighted when music elevates an otherwise enjoyable experience to something really immersive. Nier was like that for me (I enjoyed the hell out of that game, even if no one else did).
I know it's really cliche, but Akira Yamaoka is probably my favorite musician for games, even though I am actually not a big Silent Hill fan. He just does such a great job and has a very unique sound that suits his subject matter perfectly.

The original is great too, but I just love metal so much. It's intense and pretty at the same time. Never got bored of it.

Personally, Dynasty Warriors 4 features my favourite video game soundtrack.
It features tracks that go perfectly with what is going on in the game and at the same time is catchy enough to listen to separately from the game.
The soundtrack blends modern rock and metal with more classical Chinese instruments very well which complements the fast paced, "kill everything" action of the game with the Chinese setting. Ranging from "charge forward and destroy them!" [http://www.youtube.com/watch?v=uYfOcEGX7Z0&feature=related] to "Crap we're getting our asses kicked! Run!" [http://www.youtube.com/watch?v=3mz9vIGoicA&feature=related]. Chrono Cross would also be one of my favorites. Next time you go to the beach, bring the soundtrack and just lay on the sand listening to the waves and the music. It's perfect for that.

More recently indie hit Bastion has worked its way into my heart. The bluesy-western-rock soundtrack is an absolute joy to listen to. It's the perfect background music.
Also worth mentioning is the BlazBlue series. The heavy metal/symphony blend is great, even if I would have worked more 'symphony' in. But it's a fighting game, so this fits perfectly.
