I cannot beat Super Mario Bros. for the NES.
I beat it once, but couldn't repeat it.

I have never beat Mario brothers 2 or 3 though.

Also, I used to own the DDR home version.

I never finished Half-life 2. Not because I couldn't do it, I don't know why I stopped. I think it was when I got the xbox...

Cheated like a bastard on Jedi Knight 2 single-player, and have never completed it honestly.
Downloaded I Wanna Be The Guy and made it to the third screen before giving up, yes it's hard as all hell, but really... THIRD screen? Shame on me.

The first time I played WoW I didn't know that "the tram" existed. After getting a quest as a level 8 human in which I was suppose to get to Ironforge I search for a way to get past the mountains.
My solution was to swim from Westfall to Menethil harbour. This took about 4 hours and there was still some dangerous running to do before reaching Ironforge.
To be honest that sounds like a fairly awesome epic journey to me. Mind you, I'm the kind of person who flies across China in a Cessna in Flight Simulator, so maybe my mind works differently to other people's
World of Warcraft running quests are pretty epic, especially at the lower levels. The issue is that there is no stealth factor. You basically end up in the middle of a field trying to estimate the aggro lines for all of the enemies. Rouges of course have invisibility, but that is one class.

I mean early in the game, there are several other stealth abilities later on.

My big shame is getting my ass kicked on the easy mode for Red Alert 2. I was like 8 when I tried it. I remember that I only got stuck on this one mission where you have to get spies into nuclear missile silos.

Haha, I got to the 5th!
... Yes, I'm actually proud of that.

OT: For Demon's Souls I look up the strategy on each boss before I even fight it... Even then it usually takes me about 3 tries, and I don't even want to think about how long it would've taken me if I were to figure out the strategies myself.

In my female playthrough of Dragon Age: Origins, I slept with Zeveran, then Alistair, and THEN Liliana. Slightly ashamed that I ended on the woman, but other than that, it was pretty BA for me.

I cannot finish the first BioShock. It's too painful, it's like pulling a tooth. I hate that game. (I've gotten to the super-awesome mid-point 'n' stuff, but I can't force myself to go further).

Back when I played Jane's US Navy Fighters '97( an old PC combat flight simulator) I could never figure out how to land on the carrier. I'd always eject while near the ship or use the quick end key to skip the whole landing process. And while playing Microsoft Combat Flight Simulator II(a world war two flight sim) I ended up crashing a good deal, either flying into the ground in the midst of a dog fight, misjudging a bombing run on an enemy ship (and turning it into a kamikaze run) or going into too steep a dive and having my plane's wings rip off or plow full speed into the ground.

I'm stuck on Portal - I know exactly what I have got to do but can't do it fast enough

I never get any great distance in Morrowind without modding it loads (chest of good items, over-powered birth signs etc)

I always max the skills of any new Deus Ex character I create and fully upgrade any augs I get (i'm sorry but it sucks not being able to accurately shoot someone more than 5 metres away is stupid when you're an augmented human)

I have only truly completed about 5 games

The first time I played through Oblivion, I didn't realize that you can cast spells.

Bought the rifle in Dead Space, played through a few games first time on easiest setting (I still do if I really either want to feel like a badass, don't have a lot of time in the coming week and want to get through quick, just want to see the story), bought a movie liscened game, bought Chaos Legion, haven't beaten FF6 yet (seen it done by a friend who was kind enough to play through the end boss, though I suspect he did that to show off that he could more than anything else), and didn't finish Okami yet.

I tend to plow trough some of the areas in Splinter Cell games and only after killing all the enemies there do I see how I could have been sneaking around them.
I kill them anyway. Stabbity stab stab.
But what about the very pleasing feeling when you have made it trough a map without harming a single person?

First time for a laugh I went through MGS3 on Very Easy. It was way too easy with the correctly named EZ Gun.
Just FYI - that thing doesn't eliminate footstep noise. I tried.

Oh, thought of another one.

The game: Geometry Wars.

When I first started playing, I didn't realize that the right stick controls your weapons. I lost three or four rounds because I kept hitting random buttons trying to get something to happen without any effect. Once again, it was an accident that led me to figure out what I needed to do.
