Hardest dificulties that are ACTUALLY BLOODY HARD!

The original Gears of War was fucking insane on "Insane" difficulty.

Trapped in a really dark research facility full of swarming melee enemies that explode upon death and do massive amounts of damage? No problem. Suddenly stuck in a moving minecart with aforementioned exploding enemies dropping on top of you as insta-kills? Okay, bothersome. All of a sudden the minecart's picked up speed and the explodey enemies are coming at you on the ceiling? Well... fuck.

Every individual enemy on Insane was more than capable of easily killing you. Many enemies had insta-kill attacks and would aggressively use cover to get close to you and use them. EDIT: in fact, ALL the enemies had insta-kill attacks if they got close enough. The night-level with the Kryll was hard enough, but even on Co-op the later levels were so insanely hard that many people never completed them.

Oh, and the final boss was nigh invincible on Insane. Defeating him on single-player was hard enough that to this day, Gears fans brag about beating him in LESS THAN TEN TRIES on second and third playthroughs of the game.

Sure, not nearly as hard as some games, but "Insane" difficulty was still appropriately named.

On master? Christ, the game wants to kill you for existing.
Master on AMK or L.U.R.K. will get you killed so many times while being so fun that it will perplex you.

1st Call of Duty on veteran. 2-5 shots and you die (and no regen health unless you pick up hp packs). It was a long time ago so not sure on the 2 shots part though. Haven't beat it....I don't even think I beat the first level on veteran.

Demon's Souls doesn't really count since there is no adjustable difficulty.

Devil May Cry "Dante must die mode"
*procedes to cry into monogrammed hankerchief*

Devil May cry is hard enough (and you have to unlock easy mode) (fun fact: they had normal difficulty in america's release japan's insanity difficulty) but when you crank it to the max.....OH GOD MY SPLEEN! I CAN SEE MY SPLEEN! and so forth

I honestly found Kingdom Hearts 1 to be increasingly difficult, especially because I got all the choices at the start which make the game HARDER. I was stuck on that second battle with Riku in Hollow Bastion for months!

However, on a more recent note, I would say Demon's Souls (I know, unimaginative)

I recently kill Phalanx, get my body back, happy days!
Me: Hmm, what's out there?
Dragon: F**K YOU *fire breath*
(Dead, must kill another Demon to regain body and privileges)

Any Left 4 Dead 2 map on the hardest difficulty with Realism mode on. The best part about that game is its hard AND fair in being hard. I thought the first game was far to easy.

Some of the harder parts in the Rock Band and Guitar Hero series's on expert too. From Through the Fire and Flames to Visions and Caprici di diablo, them games got some of the best straight up challenge of any game.

Chaos mode on God of War 3. Gave up after I failed to beat Poseidon for the umpteenth time.

Oblivion and Fallout full difficulty is ridiculous...HOWEVER
I have completed Mile High Club from CoD4 campaign on Veteran...that was a *****.
In general CoD campaigns are difficult on veteran (no shit sherlock). I think the CoD campaigns deserve more credit.

I broke Ninja Gaiden II in half because I couldn't get past the first level on the hardest difficulty. It's the only game I have ever broken.

Not entirely proud of what I did, but I never had the urge to play it ever again. (nor could I really)

I think I remember that the hardest difficulty of Dead Space 2 would send you back to your last save point if you died.

Also you could only ever save 3 times. In the entire game.

I was too scared to even start that, just because I'd be afraid of getting cheaply killed and having to play the last few hours over again, then completely forget about where I got cheaply killed, have it happen again, and repeat until either my sanity or the game disc is shattered.

Doom's Nightmare, just for a classic
Diablo 2 on Hardcore mode is also painful.
left 4 dead on Expert always kicked my arse so ridiculously easily as well.
And yeah, because someone else said it, Serious Sam on 'Serious' mode is just... Well, evil. But I guess Evil Eddie didn't sound good to the marketing board in 2000...

This is the only game I've 100%. Veteran difficulty took me about four months for the whole game. Bloody awesome game, though.
ok, are we talking a console release, or the pc. Because i plunged through veteran on CoD2 on the pc at a daily basis when i was 10 or something(didnt have internet back then, and only game i had)
