Can't think of one that tops them all but I think:
- All of Bioshock on Hard
- All of Mass Effect 2 on Insanity
- Getting all combat medals in Arkham Asylum
- Last boss in Prototype on Hard
- All of Halo 3 on Legendary
- Last boss in X-Men Wolverine on Hard

I wanted to play real sneaky sneaky in that game so I specialized in pistols, which turned out to be the best boss killer choice in history. Chain Shot, makes everything real slow-mo and let's you line up like 5 shots in the head of the boss. Sit and wait for recharge, repeat! ^^

Can't really remember if its the hardest, but the God of War 1 God trials (or whatever they were called - the 10 challenge rooms you had to beat at once at the end of the game to get alternate costumes) were pretty hard. Oh and I did it when i was like 13 which may or may not have factored into the difficulty.

i feel like im the only one who didn't have any troubble on the ferris wheel level, everyone complains about it, but i just placed a ton of claymores and hid behind a tree and sniped everyone, took me about three tries. (this was on veteran) however i never bothered with Mile High Club, it got way to boring to complete
A lot of my tries were from the half-way checkpoint, except I had placed the mines and C4 pretty badly. Problem is, to take it back, I had to restart that entire horrid level.

I'm usually pretty terrible at defense sections like that.

And I know how you feel. I didn't have hardly ANY problems with Mile High Club despite how notorious it is.

Exit Path 2, just one moment with a bunch of swinging axes and stuff. It may be a flash game, but damn, was it challenging.

Fuck you. Fuck your 7-foot long katana. Fuck your Meteor spell. Fuck your insta-kill Sin Harvest Angel move. Fuck your teleporting. And fuck your teleporting directly behind me to give me a 7-foot colon cleaning.

Also, fuck the boss rush mode in Super Smash Bros Brawl. That was hard as hell, too.

Not hardest but most time-intensive. Defeating the 8 unlocked Pringer X Go! in Disgaea 2 or never getting hit in the mega man games...without an emulator - you get hit you die and reset the level (self-imposed challenges).

I have to agree on Devil May Cry, I obsessed over completing all 4 on Dante must die. The 3 floating heads boss was probably the hardest.
That or MGS2 Fatman on European extreme

Since I only achieved it this morning and still happen to be in a bragging mood: finishing Uncharted 2 on crushing difficulty, the monastery (oh god the monastery) was surprisingly do-able in comparison to the Tibetan village, although I still don't know why.

2nd would be Batman: Arkham Asylum's fighting trophies. I have 7 trophies left (within the freeflow challenges) to Platinum the game on... Yeah, it's getting tough now.
Well that reminds me: I got all of the achievements in Arkham Asylum. All I remember is getting very angry at a number of the challenge rooms.

May sound like nothing much to some but I VERY rarely play a game on a harder difficulty, mostly because I suck and I'm only in it for the experience and story. However I've played all the Halo games so many times that I felt a challenge was in order.

Damn I loved this game SO MUCH. But it kicked my ass SO MUCH! I can't tell you how happy I was when I finally completed it! Stupid test of fear and on-rails shooting sections!
Oddly enough, I thought Scarlet was really, really easy.

OT: Uncharted 2 on crushing...because the last boss was fucking cheap.

Beat Ninja Gaiden on NES, Ninja Gaiden on Xbox, Bit Trip Runner, Bit Trip Beat, Portal 2 co op by myself, the last boss in KOTOR (wasn't leveled enough and had to use system exploits, and Matriarch Benezia in Mass Effect (same problem).

It took me something along the lines of two hours to finish it. That might not be long at first, but remember that I retried a level that only takes a literal minute to beat 120 times before finally getting the achievement.

I also tried Black Ops on Veteran. EEEEKKKK!!!! Treyarch is the devil when it comes to difficulties. I never did beat it, though.

Most recently for me it has to be 2 player Insane GoW3 finale.
Outright, fucking solid; and I did mile high club; Fallout 3 on Expert; Halo 3, ODST & Reach Legendary solo & co-op; both Bad Company's on hardest (piss easy); & CoD 4, 5, 6, 7 & soon 8 on veteran (I say soon because the games are shit and samey so it's what I can accurately expect), they're just the FPS'.

Lost planet 2 is pretty tough, but it's boring and shit. Other than that games seem to be getting easier. I don't want games anymore that are soul destroying (Dark Souls, that's you), but games that should always keep you alert, concious and just overall weary of every second, or you fail. Sometimes I feel like I can switch off mentally in campaign and still get though unscathed on easy/hard.

Skyrim looks to be a good exception though! Whereas BF3 can be punishing if you don't stay within those 3 categories.
