Alpha Protocol almost did with its (surprisingly well-implemented) "orphans made" count as well as having enemies that were genuinely just guys doing their jobs and weren't in on the conspiracy, though, my momentary regret was quickly replaced with "this run, I'm gonna see how high I can get that number."

Every Fallout game ever... every one of 'em has potential mixups where people will attack you over a misunderstanding, and you wind up having to gun down folks who were just trying to defend their homes or livelyhoods.

on a more serious note, in almost every game I play I usually tend to take the sneaky/non-lethal way; like in the MGS series I always use the tranq gun. even in crysis I felt sorry for killing those stupid-looking npcs, in the end they're just doing their jobs and they don't stand a chance against you.
also in WoW : Festergut and Rotface in ICC; they act like children during the fight. I mean wtf!

I was doing the NCR questline and felt very guilty for having to kill Mr. House, especially afterward I get a message saying "Mr. House is dead. His grand dreams for Vegas will never be realized"

I don't know if this really applies, but I have had a lot of trouble in recent years taking the "evil" route in some RPG games like Mass Effect and Fable 2. The "Bad" routes are just so over the top sadistic in those cases that I just...can't make myself do it. So in those instances, murdering the good guys just for the sake of being literally as big of a dickish asshole as possible makes me feel bad.

I felt bad for Saren at the end of Mass Effect 1 when I went full paragon and talked him down originally. Even though he waxed himself, I still felt sympathetic to his plight for what Sovereign did to him, which is why I TOTALLY FELL IN LOVE WITH THE GAME. o_o

When I finally reached and met him, seeing him not as an evil boss with an HP to drain, but a man who followed his dream and ideals, even if it lead to the distopia that is Rapture. I had no real desire to kill him at that point...
And that's when the game so brilliantly and shockingly takes control out of your hands. One of the greatest moments I've ever experienced in a game.

I have felt bad killing some soldiers in FPS games, when they are definitely going to die and they don't just surrender and stop shooting at me. Just stop shooting and I will not kill you, but they never learn.

Medal Of Honor, which for a mediocore game actually had one of the most emotional stories of any modern warfare FPS I have played. There were to time where I felt touched (not in the way your drunk uncle touches you, the good emotional way).
1. When you are playing as Dante and you are holding off the Taliban and you are getting over run and your friend radios the HQ to call of the backup because you are done for. This bit made me think about what it must be like to be in a situation like that, where you just have to give up.
2. When Rabbit dies at the end. I have to admit I shed a tear or two when that happened, because he was fighting so hard to stay with us and his friend's were doing all they could to try to help him. :'(

Actually, that whole game was messed up in the head. Without spoiling the whole game, killing children (and the reason behind it) is kinda horrific. Though I didn't feel pity when I murdered that entire mansion after they
drowned my prostitute
Feel... bad about killing?... Impossible! At best they'll get an "awww poor bastard didn't realize I was coming, or they're just damn stupid to face me." while I grin deviously.
'Little King's Story' on the Wii. (and Pikmin for similar reasons.)
Some of the Oni feel like they're just trying to do what they're told and live their lives but I have to keep expanding into their territory. I also feel bad for getting my men killed. It's great the way they're so grateful if you manage to revive them on the beach but heart-breaking when they hold a funeral if you're too late to save them (all because you were too busy or forgot). Little King's Story is a surprisingly mature game for its presentation. The egg maze story about the guy who spends his whole life trying to make decisions until he dies is very dark.
Also Shadow of the Colossus obviously. But that's really part of the point of the game.
