The original Metal Gear, I accidentally shot a hostage while fighting a boss and didn't have enough C4 to blow up Metal Gear (nor could I get anymore).

While playing Morrowind for the first time, I lost my way between some mountains and got myself surrounded by a group of cliff racers. Then my only weapon broke.

I have played that game over dozen of times and thats not true. Maybe you were just too low level?
More like too high of a level. Monsters/bosses scale much faster than you do in FFVIII. The higher your level, the harder the game is. (I beat the game at level 8, VERY easily. The game is literally unplayable at level 100 unless you abuse the infinite invulnerability Limit Break thing.)

Fallout New Vegas did this to me and I don't understand why. I got my character to about level 17, high enough to deal with most threats relatively easily and specialised in guns just to make sure but one of the main story missions had me go somewhere infested with Radscorpions who would rip my shit up everytime I went there.

Star Ocean: Second Story, or whatever the subtitle for the PS1 was. I used up every item I had on a boss fight, and then saved at the nearby save point, assuming it would just auto-teleport me out of the dungeon as it had in the past a'la a cut scene - NOPE! I had one character alive, no (health/mana) items, and was about 20 hours into a game I still have saved on a memory card somewhere at that damn point.

I also got really close in Oblivion. I left a store that then locked behind me with vampirism. Now, as you know, that game auto-saves pretty much anytime you so much as look at a door, so I was outside, in the Sun, dying quickly with nowhere to hide. Eventually I did manage to run, while spamming heals on myself, to a nearby house, but it was close!

i actually had a situation like that in a newer game. it was in hunted the demon's forge and there is a puzzle that to solve u need to use projectile spells on the platforms to cross. halfway across it i ran out of mana and had no mana potions to begin with. so i was stuck in the middle of this gorge with no way to get back and had to restart from the last checkpoint :mad:

Paper Mario. The first one on the N64. *spoiler alert* Bowser was rediculously hard at the end and I didn't have enough items to survive the backtrack to get to a good grinding area so I had to keep fighting him over and over. I gave up for a week or two before picking the game up again. After an insane amount of tries, I mastered the timing thing where you have to hit the button before you land on them during a jump attack to bounce up for another hit without giving them a turn to attack. FINALLY BEAT THE -insert_suitable_acronym_here- Koopa after spamming that attack like crazy. The ending cutscene was worth it. ^0^

Secret of Mana. Firt time i fought the mana dragon, he simply disappeared and never appeared again. Couldn't attack him and I couldn't die. It was quite annoying.

Morrowind comes to mind, a couple of times. Often I kill someone essential to the plot, but the game helpfully tells you that. Doesn't tell you if they're essential to a side quest though, which can be awkward.
I have also sold essential items, then had to check every merchants inventory until I found it again.

One of those earlier Leisure Suit Larry's. I can't remember what, but I failed to pick up all the items and was unable to progress. In a number of games I quicksaved with little health just before getting hit by a bullet/sword/spell/giant monster body part while having no proper saves. Sucks

That, and also I couldn't get the real ending for Prince Of Persia: Warrior Within because there was a life upgrade which you can't get again after you've finished the game, and I didn't want to replay the game, so I watched the ending on youtube. Also, I almost got stuck a load of times in half life because of awkward auto save timing. Mostly it was just because I had low health and/or very little ammo (mostly at the start of the game), but one time, I jumped off the cart and landed on the electrical tracks (taking damage), and the game auto saved just as I jumped. It took me like 1 hour to get past the level (due to heavy damage from the rails, and I didn't have very much health in the first place).

Edit: Also, I can't seem to get Prince Of Persia 2008 to work since it crashes on the first cut scene (where Elika's dad cuts down the tree) every single time. I don't know if its my computer or the game, so I'm not sure it counts.

In oblivion there were several side quests I was not able to complete because NPC's necessary for them died. Like Gunder at the Colovian Trader in s Skingrad.

One day, I was carrying too much stuff to move on my own, so I needed to wear the Graw Cowl of Nocturnal to carry 200 extra pounds of stuff, so needless to say, the guards chased me as I ran into the Colovian Trader to sell my stuff As I run into the Colovian Trader, Gunder darts out of the shop (because I'm the Gray Fox), and I take off my cowl and yield to the guards so they leave me alone, and then the guards see Gunder running and kill him. FML -_-

And lolz at the people who couldn't figure out how to play the game and died because they developed their character wrong. xD

Fallout: New Vegas, my second run through.

It got about halfway through, after I'd finished Dead Money, I realized I was playing a really awful game and couldn't physically finish.

Pity.

Several times among them: The classic save in the wrong time/place (you have 2 sec need 5 min..). Avoided (by mistake) story trigger. Used cheats to unlock doors etc before they should be possible to open ->crash.. And o yes Oblivion certain gates etc does not show up where they should.. and so on

Name of the games, no freaking clue these where some time ago and my memory is not that god on mediocre games which just made you pissed by crashing..

The final boss in Sonic Unleashed. Before the final boss, you go through this ridiculously long and difficult final level that takes expert players 20 mins to complete (it took me about 30 min). After that ghastly ordeal, you are locked into a 3 part final boss fight.

The first is a laughable final confrontation with Dr. Eggman (unless you suck at quick time events). The second is a strange shift between coming close to the final monster and Sonic running towards the monster's weak points. The problem is that while getting close to the monster, he hurls things at you and you need to get close to him without losing all your health (yes, there's a health bar involved). By the third attempt, is nearly impossible to get to the monsters and proceed to the third and final part of the boss fight (Super Sonic). However, if you fail (die), you will need to start all over from the beginning.

If you lose all your lives, you can return to the main map and chose to fight the final boss or play another stage. The only problem is that I don't know if when I chose to play another stage, I'll need to complete the final level again (and I definitely don't want to go through that again!).

So I'm forever stuck in a Sonic game.

Oh and that happened (minus the weapon breaking), but I managed to get to the town by finding a way to "climb" the rocks, and managed to escape to Balmora.

Not true. With proper leveling the enemies get easier as you level up. Are you sure you're junctioning like you're supposed to?

Playing the MW2 campaign, one of the favela missions on Veteran, and a checkpoint saved when I was stood next to a car that was 0.0001 seconds away from exploding.
Result: Endless deaths, restarts until I got bored of trying and hard to start again

In Oblivion I had the vampires disease and i foolishly fast traveled to on of the towns and when i arrive at the gate it was day time the game auto saved and i died instantly so now on that save i just die every time i load it up
had to make a new game :(
