make sure to make eye contact. Its a sign of confidence.
Ask in a controlled, mature, quiet manner (no cracking voices from fear)

and remember, if she says no, smile and reply with "Ok cool. Thanks anyway" and walk.
Leave like a man and she will remember you. Thats a positive step.

I just ask. I never really consider dates very serious, so I usually just call them up and ask them to come out to do something with me. If I don't know them well, then I'll usually say something about how nice it was to meet them the other day, and if they're not busy, would they like to come see a film with me? Or something like that.

If I'm doing it in person then I arrange to meet with them again for a 'date' or something just as I'm about to leave. I don't like asking mid-conversation, because if they say yes, you've still got to keep talking to them, and same goes if they say no. Honestly, just ask. The worst that can happen is that they say no.

Charge in, chloroform rag held high! If by chloroform rag, you mean willingness to ask someone if they'd like to go for coffee or something sometime.
Then if that goes well, arrange to meet again in the future for something that's more obviously a date and/or go for the remarkably cheesy "So, um... Do you want to like go out go out?"
Quite how that worked I don't know. Helps to have a girl that really likes you back, it seems.

Well, I used to get to know the person a bit, then I just asked her. If she said no, I moved on. But, I haven't done this in over three years because I don't date anymore. I just don't see the point.

I ask people out by saying: "Hey, are you busy this weekend? Because there is a (insert anything you could do on a date), and I was wondering if you wanted to come with me."

With any luck, she will say yes. If she says no, than no skin off my back. Hell, maybe she said no because she wanted to do something at a later time...
