Left4dead. Deadrising. Or is you REALY want to go underground into some small corners of the interweb, brainbread. Brainbread is an old, small half life mod. Everytime you shoot something it explodes into bloody gibs. Also if you take off the head of a zombie a fountain of blood shoots out the neck!

I would suggest God of War.. Assassin's Creed has some pretty violent death combos too, but God of War takes the cake.

Can't really help with a truly bloody game, haven't really played alot of new stuff, limited budget and other interests. But from what bloody games I have played, while it does add realism to the game, there comes a point where there's more then what is really necessary, such as in mortal kombat, you punch a guy once and 3 puddles of gore appear on the ground, unrealistic and overall unnecessary.

I must say that I have a certain love of gore and blood in games (not because I'm some weirdo who get's off on it but because it just adds a certain 'over the top quality' that I find funny and like).
I'm not overly bothered if a game has little or no gore though (it's just a bonus if it's there).

so you want a first person shooter specifically? If you're just looking for something that's not clunky like most 3rd person shooters, Dead rising had a lot of blood and decapitations (you can reach into a zombie and tear out its intestines,) and is nothing but sheer mindless fun, with good controls and awesome gameplay.

Did you forget to take some medication this morning? Isn't manhunt kind of a game like what you're describing though? The most violent game I have ever played is definitely Fallout 3. That game eventually makes me sick of the gore. I took the bloody mess perk once to see how it works and it makes things pretty ridiculous.
I'm sure that there must be a straight on gore fest out there somewhere, but you're beginning to sound unstable to me. I don't want another killing spree and then more blame on video games.

How about Postal 2 with the A Week in Paradise mod? I was recently playing with that mod and every shot put a pretty good sized blood splat on the wall or ground behind my targets. Headshots cause a fountain of blood to spray everywhere, if you do it with the shotgun the head comes apart and chunks go everywhere, explosives can blow bodies in half, get yourself a copy if you can, may be banned where you live and have fun.
