Anyway, the worst reason I've been kicked was way back when Guild Wars first came out. It was my first play-through so I wanted to watch the story cutscenes during the mission. The group leader wasn't too chuffed with this as this was apparently his 3rd play-through, so when the cutscene ended he kicked me. This is one of the many reasons I prefer single-player games.

I feel your pain my friends. I've also been kicked for giving advice (in team games no less), helping newbies (sometimes newbies on said team), taking a break while sitting in the lobby to grab a drink (I announced my intentions clearly before leaving), and having a username someone didn't like. That last one may have been the worst since it was my PS3 name, B_A_Start. Who doesn't like references to the Konami code?
