The missions in Guilty Gear xx Midnight carnival.

Fighting someone with regenerating health?
Fighting someone while your health depletes?
Fighting someone who has a full special bar?
Fighting someone who has to be hit with atleast a 10 hit combo before they take damange?

i know, lets try all those effects at once! see how you like that!

Stupidly Difficult

and the Meat circus on Psychonauts, the worst bit was jumping around that spiraling net thing with fire on it.

Star Wars Podracer for the 64. The computers in the later races somehow magically gain the power to find and pass you. No matter what shortcuts, tricks and skill you might have.

For me, it's definitely 'Playing Judas' from Freespace 1, but by extension I also would like to include ANY instant-fail stealth level from ANY game ever made.

And then the final boss. I could beat his first round pretty consistently, but then the second round, I could land maybe TWO hits and then he would counter-spam and rape me with his version of Shoryuken.
Seth brings my piss to a boil.

Another issue I find with grab characters is that they can throw you whilst on the floor.
