im asking this because i just beat the game and lost 5 squad-mates(Legion, Thane, Grunt, Jack,and Mordin) and i was wondering if anyone else had a similar experience.
Not only did my entire squad survive, but I saved my entire crew as well.

Are you bad enough a dude to achieve the same result?

I've never lost anyone, but my boyfriend lost Legion by using him as the tech specialist. Not sure what he did to do that. I didn't lose anyone, even my crew. I refused, like others have said. ParagonFemShep don't mess around. I have seen youtube videos where someone got EVERYONE killed, and Joker had to deal with it all. Made me lol.

My fist time through i lost legion i was pretty pissed cuz legion is cool but apart from that the rest of my playthroughs i didn't lose anyone

your only option is to pick max paragon or max renegade. Sorry, but its true. I sided with Jack, though, and told Miranda to grow up. Even having lost her loyalty she didn't die on me, though.

My first time I lost no one but it was on normal and as long as you choose sensible people for each job its easy as pie.

Nope, I've yet to lose anyone during that suicide mission. I simply refuse to leave anyone behind.

First play through I lost Mordin, not sure why but apparently hes the most potentially fragile.
(came off a ME1 save)

Second play through I lost no one, basically rectified my mistakes with Mordin.
(Came off a different ME1 Save)

Third playthrough (PC, the other two were 360) I lost half the crew and Miranda, because I failed to have enough renegade or Paragon when she and Jack got in a spat. The crew were because I just completely forgot to get a loyalty mission early on, and went to the final confrontation late.
(No ME1 save)

I'm playing a new ME1 character and wondering if I shouldn't personally sabotage my crew to see how different itll be in ME3.

My first time through I lost Garrus [sub]and Jack[/sub] and I was crushed T_T. Garrus was one of my favorites. In fact, he may be my NO.1 favorite. Needless to say I saved everyone's ass the second time through. EDIT: What I want to know is what ME3 will do if you got the ending where Commander Shepard dies.

my first playthrough I had lost Legion, Thane, Tali, Samara and Mordin, and lost some of the crew being late to enter the relay.

It's a shame because I liked Mordin (and Legion, but never used him that much during my playthrough)

first try I lost legion and mordin, I couldn't tolerate losing mordin so I reset and got a flawless run.

OT: I played like Shepard = The Doctor. So, naturally, the second I thought someone died, I shitstormed the whole thing. Ran straight ahead, guns blazing through the whole last level and used minimal cover for the final boss fight, steady stream of bullets basically the whole time. Then I found out no one died. Felt good, man.

No, no, that's the Doctor alright. Eccelston, Tenet, and Smith, yeah, they all get pissed when you hurt their friends.

OP: Never lost a single teammate. I typically go for 100% completion, so I do all the side quests before story missions. Got everyone loyal before getting the IFF, and mined enough resources to upgrade everything.

I lost my entire crew, Kasumi because I hadn't done her loyality quest, and Tali since I decided to tell everyone what had happened, at the trial.

you need to have it high enough before their encounter and use it then and there. Basically the option is the same either way, you say something along the lines of "This is my ship, you all do what I say" and they both back down.

I think that leaves Tali, Garrus, Thane, and Samara left in my crew along with the Normandy crew.
I don't understand people like you. The loyalty missions are literally the other half of the game. Why would you purposely not do half of the game and just screw yourself out of content you paid for?

There is a known bug that the fight can't be resolved if Miranda's loyalty mission is done before Jack's. Same thing is true for doing
Legion's loyalty mission before Tali's
You need > 75% paragon or renegade to resolve the fight which is easily obtainable until that point so if you got problems there nobody can help you.

As for the OP:
Here is a guide to save everyone

1. you need to pass all loyalty missions [you actually can fail Thane's so care]

2. After aquiring the reaper dial device you have exactly ONE more mission before your crew will die (Kelly etc). You will need to use this mission on Legion's loyalty one. Remember that every single loyalty mission but Legion's needs at least one mission in between recruitment and loyalty mission trigger (sidemissions do count). This means your last mission before the reaper cannot be a recruitment mission.

3. You need all shipupgrades:
-> Weapons (Garrus)
-> Armor (Jacob)
-> Shields (Tali)
-> everything else is optional

4. Choosing the right team:
There are several options here but since i never read a guide i only know the one i use:
-> Tunnel: Legion
-> Squadleader: Garrus
-> Biotic expert: Samara/Morinth
-> Squadleader 2: Garrus
-> Escort: Jack
