Mordin died for reasons unknown, something about how the game computes the defense of the team you leave behind during the boss encounter. I had a sad for some time ;^;

1st playthrough, just Tali died; she was my love interest so I was devastated (well, ok, irked). As soon as I finished it, I reloaded, made the correct decisions and everyone survived. (Everyone was already loyal so just had to choose the right people for the suicide mission).
Jack, I tried everything in my power to get the highest paragon level just to tell her to shut the fuck up. I even waited for her loyalty mission to be last. In the end, she is the only one who died. Why? She's a pain in the ass. I didn't like her at all.
On my first playthrough I purposely got the bad ending (killing everyone), on my second playthough I managed to save everyone.
