On my first playthrough it was Mordin and Jack that died. Then my second i rigged it so no one died.

No one dies on Steve Roger Shepard's watch.

I have found it useful to take either Mordin or Tali and send the other one back with the crew.

This is my exact experience, except I was fooled by Zaeeds utter uselessness.
Couldn't you tell from dialogue with "I am the sole surivivor of dangerous missions, because I got all my comrades killed" Zaeed?

Nope, not a single one, except on my second playthrough where I wanted to see what would happen if everyone died... it was pretty sad, and it really made me wonder what will happen in Mass Effect 3 if Shephard dies

EDIT: And I actually got the captcha right on my first try
the word in the captcha that doesn't look 'double' can be replaced with anything, it doesn't matter what you type. They're using it to digitalize books. I for one am sabotaging this by replacing the 'free' word with 'Hitler' every single time.

Miranda. It made me happy as I was sick of her and sick of the ass-shots at that point. It made me nostalgic for the time I found out I had it within my power to send Ashley to her death, and laughed with maniacal glee as I sent that ***** off to the slaughter.

First play through, I lost legion.

The dip-shit stuck his head out from behind the door and got shot in the face.

First Legion died when I sent him to the vents or whatever. After the Reaper fight, I lost Miranda at the end. I was kinda pissed about both. People keep saying "if you do all the ship upgrades no one dies." I call bullshit, because I'm pretty certain I had them all because I had no options to anymore. So either I messed up somewhere else, or that really is a bullshit answer.

Lost Thane that was it. Even though apparently I lost 2 more somewhere because there were 3 caskets at the end of the game. Only reason I lost Thane was because it said it needed someone that could lead a squad and I was like Hey he can do it. Apparently only Jacob could do that part which was stupid.

Didn't lose anyone in the end, i reloaded when the super experienced ultra warrior (Samara)ended up being a poor fire team leader. Couldn't stand to lose anyone :-(

No one died on my first playthrough. Was a bit worried about Zaeed, since I didn't get his loyalty, but I was fine.

Since i wanted the 75 gamerscore, i saved everyone. (Or else i would've let Jack die, since i hate her)

If I don't like how some characters are portrayed in ME3, I'll definitely be swinging by ME2 to ensure a few "accidents" happen on the SM.

Say, if Samara really does decide to come after my renegade Shepard, or if Jack goes pirate, or if Jacob is a main character >_>

First playthrough I lost a couple of people, but I went back and managed to get them all through on my second attempt.

There are some handy walkthroughs online if you're having difficulty choosing the right people for each task (and if you've made it through the suicide mission already it's not like the walkthroughs are going to spoil anything for you at this stage!)

The ship upgrades are part of it, but you also need to do the loyalty missions and choose the right people for each task.

It's a little counter-intuitive because the wrong choice is not always the person who actually dies: I sent Tali through the vents (which was fine), but then she died in a later cutscene because I'd sent the wrong person to lead the second fireteam.

I know I know, it just looks... confusing, but nothing beats the feeling of getting a captcha right when you are in doubt... yes, I am very lonely

Did all the loyalty missions also. Though I didn't manage to break up the fight with Jack and Miranda right and Miranda was forever pissed at me so I'm sure that didn't help. If there was one person I didn't mind dying it would have been Jacob, I wish I had picked him for something and had him die in the process. Oh well, whenever I get around to a 2nd play through I'll try to fix it!
