People are going to read into this what they want to hear. As a pessimist it sounds like Mr. Boon is trying to sound optimistic for his fans but in reality is toeing the line. He apparently wants to make an M rated title to hopefully restore the fan base a bit, but at the same token he's apparently worried about taking things too far and upsetting anyone since he talks about there being a line even for Mortal Kombat, and actually invokes fear of an AO rating (smoke what?) which is in of itself a sign that censorship/the ESRB has gone too far.
Simply put you can't become AO rated over violence. It's been tried (mostly with movies) it doesn't stand up. Even the more successful campaigns that were temporary successes (like Europe's "Video Nasties") ultimatly failed big time. Though admittedly Hollywood showed a degree of spine that game companies/producers have yet to develop.
Generally speaking, from waaaay back when I was in school I seem to remember a very clear standard being mentioned. To gain an "X" (or in this case AO) rating the visual subject matter (game, movie, even painting or photo) must show sexual penetration. As in tool A fits socket B right there in front of you. This was actually extended successfully to include things like oral sex, and penetration with sex toys (dildos, strap ons, etc..) which could have been considered exceptions under the original law (which was part of the point when it was coming up, laws being amended within the original spirit).
What this means is that you can show all the nudity and simulated sex you want, and nobody can force you to take an X/AO rating, though you can choose to. When it comes to "X" it's all about the "money shots" (called that for a reason).
When it comes to violence there is no limit, and honestly that has been an issue. Private pressure (which is a problem) has done a lot where it shouldn't have been able to, but if you check out the history of horror movies and such you'll find it littered with attempts to ban movies or get them rated X for violence or mere "subject matter". Any successes on the subject when met with proper defense have been temporary at the best. Movies like say "Cannibal Holocaust" are noteworthy for this... and honestly just because movies are older does not mean they did not involve shock trends like you see today. There are some movies that can be considered pretty "over the top" that aren't exactly new. Also things like the Binoculars Scene from the old Vincent Price "Horror OF The Wax Museum" (I think that's the title) are messed up in any "era".
I wish Mr. Boon the best, but honestly if he's going into this "toeing the line as close as he can to avoid an AO rating", I wouldn't expect that much. Right from the beginning it shows he's intentionally trying not to shock people enough to get attention (other than fan hype) and is more likely to run from the threat of an AO rating that actually saying "go for it" and then duking it out in court on principle to maintain the proper interpetation of "R/M" and "X/AO".
I know many might disagree, this is simply my opinion, and how I've learned things over the years.
