Definitely Johan. One of my favorite shows of all time.

Surprised to see no one mentioned Capt Martin Walker here. The whole game was a travel from his paladin, idealistic military persona to a pretty fucked up killer with a broken mind.

He's not just a plain psychopath like the others, he gets you to crawl inside his head and try to see life as he does. It's deeply disturbing. And at the end of it all you don't even fight him, he just continues to exist in his malevolent fantasy world while you're left to move on and try to forget about him.
Keep in mind, too, Sander Cohen isn't anyone special. He's just another splicer, ultimately, the same as the ones you've been killing the whole game with hardly any remorse. In Rapture, where power is suddenly everything, a calm man with an artistic vision is somehow more terrifying and survivable than an 8-foot tall armoured monster with a drill arm.

No mention of one Gregory House yet? Interesting. The guy was beautifully messed up. Clearly ahead of most in the intelligence department, but couldn't maintain friendships or relationships, self sabotaged every time something good started happening to him, it was amazing watching him implode. I'm not going to spoiler it but the end of season 5 and beginning of season 6 were truly amazing.

On a lighter note, also Max, the Hyperkinetic Rabbit of Sam and Max fame. Bunny's pretty messed up.

I'm going with pretty much every character on Moral Orel, with special mentions going to Clay Puppington, Orel's physically abusive, Oedipus-complex-having, closeted-bisexual, alcoholic father. Or any character featured in the episode "Alone"

Guts was quickly forced to earn his keep participating in full on wars even as a young child. A while later his father secretly sold him for a night to a fellow mercenary who raped him which would scar him for the remainder of his life. He never sleeps without his sword in his arms from that moment forward often hugging it in some attempt of affection. Then a few years later Guts father loses his leg in a accident and it becomes Guts job to support the two of them. One night in a drunken fit his father lashes out at Guts revealing that he had in fact sold him to be raped blaming him for his mothers death guts tries to defend himself and accidentally kills his father slicing him through the neck. Immediately after members of the band find out what happened and attempt to kills guts chasing him out of camp and off a cliff. Guts barely survives the fall and manages to limp on forward only to be attacked by a pack of wolves with nothing but him and his sword. He manages to fight of the wolves but only within a inch of his life.

Skip ahead a few years he is about 18 at this point all he knows is war and he works as a mercenary moving from war to war avoiding everyone and everything. Eventually he meets a band of mercenaries who ambush him. He overwhelms them and their leader takes notice and challenges him to a dual for his freedom. He loses his freedom but finds home. It takes him a whiles but he finally starts to open up to them. He makes friends and their group works their way up the ranks all the way to knighthood and then to the rank of noble and even to the countries top army he becomes second in command and even gets a girlfriend. After all this he finds out that his best friend (the leader of the army) doesn't respect him because he is to much off a follower. So in a attempt to carve his own destiny he leaves only to find out in his absence his best friend was arrested and tortured for 2 years. He manages to rescue his friend only to find out he can never walk talk or really even move ever again.

This is where things get a little crazy.

From here he is running away with the remnants of the army after the rescue of his friend, only to have his friend in all his depression summon the demon lords and sacrifice everyone Guts has
ever known or loved to being eaten by gruesome monsters in order become a demon lord himself. After his friend becomes a demon lord he proceeds to rape Guts girlfriend in front of him while Guts arm gets eaten off his body by demons and his eye gets pieced close with this being his final sight (at least in that eye). Guts manages to escape all this with his girlfriend but just barely with his girlfriend now having lost her mind and him missing his arm and a eye and that is the first half of the series not even the whole damn thing.

I have never seen a character go through more than guts the man is insanely complex and interesting and the story is soooooooooooooooooo well written you really empathize with all the characters and the situations. The lore is deep and complex all the while being interesting to under stand while still having more to learn with each re-read.

Probably not the /most/ amazing one I've seen but I think it's worth a mention: Lord Vanaduke from Spiral Knights. Sure we've all seen the sort of "twisted in a fiery rage" sort of character before but they go pretty in-depth with him. When his kingdom is under attack, he retreats to a mountain where an unknown force offers him the power to crush his enemies. He accepts, naturally, only he ends up burning his entire kingdom to the ground with shadow-fire

Several hundred (or maybe thousand?) years later, his kingdom is ripped from his planet to become one of the most dangerous pieces of a giant deathtrap planet, because he still believes his kingdom is under siege all this time later, and his own rage is still keeping his underlings alive in the form of fiery undead knights. He's trapped the sprites that came to try to cleanse the shadow-fire, one of his closest friends committed suicide as an alternative to being corrupted by the fires, and when you approach him all he sees is "more wolves, hungry for the blood of Almire."

Not exactly the most amazing, but now that I think of it, he reminds me of another one from Cave Story (100% completion spoilers)
Ballos, the brother of Jenka. He was a nice little Wizard who everyone loved and trusted... until the king became jealous and tortured him. Turns out, intense torture and magic are not a good mix. He got to watch as a magic blast that he created destroyed everything and everyone near and dear to him. His sister locked him up in a flying island cave. Worked out well... except he was also forced to create the demon crown, but he then cursed the person who forced him to do so (Misery... also Balrog for some reason) to eternally serve the wearer of said crown. When you finally meet him he begs for you to kill him, but his magic is so far out of his control that he still goes all out on you.

Just a heads up they start in the future then after a chapter or 2 they skip to the past and work there way forward to present day again so that is why he is a adult missing a arm and eye and such at the start.

If nobody's mentioned it yet, Kazuo Kiriyama, from Battle Royale. An eighth grader who has, in his entire life, not felt sympathy for anything. He's a single-minded, psychopathic MACHINE, and comes very close to taking the entire game. Particularly his expansion in the Manga (such as his scene with Jaguar *shudder*) made him seem even more impressive than he looked initially.

As a second, Mitsuko Souma, from the same book. Battle Royale is a good read, if somewhat strangely written in English.

I never played all the way through Bioshock (I should) but I still remember that scene... Enraged at how his current patient is turning out all wrong... meep .-. "It's all wrong! *stab*"

Also: Someone mentioned Deadpool before but they didn't really go in-depth. Sure he's somewhere between medium-aware and schizophrenic, but what's even more messed up is WHY this is the case. Turns out his family had a history of cancer and he got to watch his mother die from it, and then he developed cancer. Then he was experimented upon and given Wolverine's regenerative abilities, which makes him very difficult to kill. Unfortunately, the same regenerative abilities also affected his cancer, which caused his face to become a deformed mess. To top all that off, his brain cells regenerate rapidly and unpredictably due to the mix of cancer and regeneration, which is why he's so insane.

Also also: Several people mentioned Deadman Wonderland, but didn't mention the two biggest contenders from it so far:

Nagi, who carries around a locket with a picture of his child who he promises he'll see when he gets out of the prison... Except the locket is empty, because his pregnant wife was killed by one of the undertakers. He couldn't even scream out for her when this happened because his vocal chords had just been removed. His insanity nearly allows him to be turned into an Undertaker and he nearly kills all the other people he loves
and
Genkaku, leader of the Undertakers, who became convinced that he was saving people by killing them, and ended up "saving" his master shortly after coming to this conclusion. This eventually leads to him flipping out in the middle of the final battle and killing all of the guards in the room.

God I loved Ladd and Claire/Vino so much in Baccano. Neither of them were immortal but had some serious mental issues that put them above most other characters.

Seriously though as fun as it is to watch Ladd, I gotta admit Claire was just as entertaining at times too. How he can be so laid back with whole Railtracer persona and his method of torturing that kid....*Eeesh*
Plus he confessed his love to Chane right in front of Graham Specter as they were fighting.

If we are going from TV, I'd tentatively say Derek Hale from Teen Wolf? Dude gets into a relationship with a mid-twenties year old ridiculously hot person Kate Argent at the age of 16, has her emotionally manipulate him into thinking he loves her... Only to have her burn his entire family to death in their home; she was simply using him to establish if the Hale's were werewolves or not, and the minute she confirms it almost everyone he knows and loves are dead. His sister and him survive, and an uncle is left severely scarred and comatose, but when returning from New York, the sister is murdered.

Derek is therefore exceedingly messed up, being a werewolf that never was able to receive proper guidance and training on his birthright, feels he is nearly directly responsible for the death of his family, and is sexually stunted, only ever showing in the series sexual manipulation towards other people. At the same time he does seem to be a decent person, going out of his way to save people who despise and mistrust him, even if he tends to make things worse through ineptitude and generally acting like a creepy serial rapist or something.
Which is why the fucked up is beautiful; it really demands the question of what kind of person he would have been if he never met Kate.

Jack from ME2. At first I was like "Why in the world would the Illusive Man even bother with this chick?", then I saw her and began to understand how awesome she was as a character. Sorry OP, she's just so awesome!

Handsome Jack and Tannis from Borderlands 2, both of whom are completely F'd up in their own way.

he is just a ruthless and vicious character who will go to great lengths to get revenge
The great thing about his character is that he is more complex than just a insane racist
Which kind of makes what he does all the more inexcusable
