Fighting Iron Knuckles on Majora's Mask (my first run through) using the Kokiri Sword or just facing the two at the same time in OOT, I know that they move slow at first but you still need to watch out and especially towards the end of their life when they get fast.

I don't think you focus on combat skills enough. A couple of shots with most of my standard weaponry takes one down (head-shots but thats ok because i combined the commando and sniper perks).

Most enemies that you don't notice until they are right behind you...

Tried a Giant Radscorpion creeping up on me, turned around to check if Fawkes was still there and BOOM! Radscorpion humping my brains! Almost fell off the sofa...

These, from Silent Hill 3. I think the worst thing about them is they're so fast in a game that's actually so slow paced there is no way you can fight them without getting half your medkits drained.
Oh yeah, and their shreeking.

Id have to go with Regenerators and Iron maidens from resident evil 4. Their not very fast on low difficulty but the way they wheeze and move is incredibly unsettling.

Other honorable mentions go to Death Claws (on early levels) mostly because your dead meat if they get up close and your not wearing power armor.

And the Stasis Slashers from dead space because of their impossibly fast movement.

I hate these bastards. One is tolerable, but killing multiple Shadows in a limited amount of time is the very reason I can't complete Devil May Cry on Hard mode.

The T. Rexes(sp?) from that King Kong movie game. When you played as the human, they were invincible.

I can take a death claw down easily now, mainly with dart gun and Fawkes, but when i run into some i still feel like running away, even though it will catch up.

Took me about 20 goes to clear the first building of those bastards. Anything that can jump like a flea, accelerate like an exocet missile and kill you with one hit on MEDIUM is, I feel, an unbalanced enemy. I was more concerned about the tank-blokes in enclosed spaces, though. In a game that took such pains to be realistic a dude that could take four anti-tank rockets to the face without breaking stride seemed like it had wandered in from Quake 4.

Any unit that can hit me from a mile away with a short-range weapon. It tends to bug me when I start taking plasma fire from Enclave troops so far away I can't even see them
