Silent Hill 2 still scares the fuck out of me many years on. I can't bring myelf to play it on my own, or at night.

I remember when I jumped into a sewer and the bastard was there, I needed to change my pants :'(

My answer will always be the same:

That is the most horrifying thing ever.

Honorable mention goes to the ReDead from Wind Waker. The ones from Ocarina of Time lost a bit of their creepyness thanks to Majora's Mask, where you can get them to burst into dance by wearing the right mask. And there's just something about the exaggerated, cartoony look of the Wind Waker ones that make them so much more horrifying.

How have Deathclaws not been mentioned yet?
Everyone's, and I mean EVERYONE'S, first thought when these pop up on screen is "FFFFFFFFFFFFFUUUUUUUUUUUUUUUUUUUUUUUUCCCCCCCCCCCCCCCCKKKKKKKKKKK!!!!!!!!!!!"

Seriously, fuck those guys.
on a similar note, Deathclaw Denmother. the bigger, stronger, harder to kill version form new vegas

Mine is from Resi 4, the simple cannon fodder zombies that, after being decapitated, GROW ANOTHER, BIGGER, FREAKIER HEAD. like a centipede with blades on it.
You tell me, when you've already blown a zombie's head off what do you aim for? all your zombie-killing strategies go out of the window and you just have to unload every bullet you have until it's dead. at least there's a reasonably simple way to kill regenerators, they're basic enemies that can take you completely off-guard

Honerable mention to a game called Dark Sector, your aiming is clunky and the camera isnt your friend, then the game drops you into a black sewer and you fight wave after wave of mutant redneck style things that can over whelm you easily. I think it helped that I was about as crap with the gun as i would be in real life...

Ghosts from Silent Hill 4. They like to follow you, and you can only pin some of them down sometimes, and even then they make horrible noises. Possibly the worst part, though, is the fact that they each have an identity. They have faces, clothes. They were individuals with lives, and now they mindlessly prey on people lost in living nightmares, possibly forever.

Honorable mention: Dead Hand from Ocarina of Time. Limbs sprawling all over its lair to grab you, and that horrible face...

RE4 Colmillo [http://residentevil.wikia.com/Colmillo]
These Las Plagas wolves are the bane of every moment I'm in that maze when I play. Regenerators also scare me because of that creepy breathing but they're easy to defeat once you get the infra red scope or one of the more powerful guns. For the Colmillo every single time I encounter them I get that shock of fear sensation when they appear and they are the ones that do the most damage to me. If the entire game consisted of mostly Colmillo instead of the other Ganado types we'd have more deaths in the game than you would missing a QTE button command.

HA! I beat the double post glitch. VICTORY!

Bloodsucker from the Stalker series.

You hear this weird panting and footsteps, you see a shimmer and start spraying the place with bullets. Thinking you've killed whatever was causing the noise a smug grin appears across your face as you reload, and then you get ripped apart by Zoidberg's evil, invisible, brother.

Sure the words 'You are dead' immediately appear across the screen and you just reload your quicksave, but thats not the point.
Definitely! When I hear their roar my reaction is usually something like this: Night vision on, flashlight on, throw grenades through every door...Okay...It must be dead now...Ohmygoditsnotdeadthereitisanditsjusteatenmyface!

wall masters (twilight princess)
brood mother (dragon age)
TANK! (L4d/L4D2)
Ghasts (minecraft)
zombie dogs and crows (resdent evil series)
Amnesia The Dark Descent in its entirety

I am serously surprised creepers have gotten a ton of mentions but none for the horror that is ghasts

The scariest I can remember was the Flying Polyp from COC: Dark Corners of the Earth. Not only were there two of the ugly, huge SoB's, I also had to fight them over a seemingly bottomless pit with the wind constantly threatening to push me off the narrow platform I was standing on.

Father Dagon was also a pretty impressive sight the first time I came across him earlier in that same game.

EDIT: Found a good picture of a Polyp (not the one in the game, but pretty close):

Tanks in Left 4 Dead, the fucking striders from Borderlands.

Also, the Helghast from Killzone 2, because there are hundreds of them, and they can all take a clip of bullets each before going down.

The Regenerators from RE4. Powerful attacks, nearly invincible and utterly terrifying to look at.

When I first heard them breathe I started shitting my pants. The thing walked around a corner and suddenly stretched his arms out and grabbed me; started gnawing on my flesh.

The pissed off chicken from Zelda games is quite terrifying, you cannot survive the endless chicken barrage.

The invisible water-monsters from Amnesia: The Dark Descent. I land in water and immediately start running when I hear those heavy "KA-SPLOOSH! KA-SPLOOSH! KA-SPLOOSH!" in the water. Never been so genuinly exhilerated to run away from anything in gaming ever.

Edit: Oh yeah this kind of regenerator is only seen once I think. The others are slightly darker, don't have spikes on them, and have a little less amount of leech things on them.

I found the Ichthyosaurs in Half-Life to be truly scary. I mean, they're pretty creepy to look at, but the main reason is that they're so dangerous. They move fast. They make that... sound (which I don't even know how to describe) when they're barrelling towards you to try to bite you in half. And you have to leave your comfort zone in order to fight them, because it's very difficult to kill the damn things from above, you have to get in there, in the murky water, where it has the advantage. Also, most of your weapons don't work underwater.

And then there was that part where you had to navigate the slippery, crumbling walkways above the pool occupied by two or three of them.

"Don't slip, don't slip"
*slip*
OH GOD
There's that one part after avoiding the chopper and swimming in the dam to that drain: the damn things get in your face! And I think it's sorta like a howler monkey's call echoing out of a tin can or something.

Also, Emerald Weapon from FF7: I never really liked underwater stuff to begin with, but I HATE the submarine section. His position is random, so he could be right behind you first dive! I've also heard he likes to hover around the crashed ship dungeon, which means he could be waiting to strike as soon as you leave. He's a hunter waiting to pounce!

The scariest enemy ever? Wolves from Penumbra: Overture. In most games, even the most terrifying enemy is just another bullet receptacle. In Penumbra, you can't do shit, and that was a truly scary feeling.
Lol, you can easily kill the wolves in Penumbra, you just stand on top of a box and wait for them to run up then you hit them without fear of dying because they can't touch you. At one point I wanted to rush through a level and I timed it right and waited for the wolf and then I beat the hell out of him with a pick-ax.

Although I can't think of any real terrifying one I guess I will have to say the water monster from Amnesia, you can't see it, you can't outrun it, it is loud as hell, it digests human limbs in record timing and you can't fight it at all.

And you run through that shit.

PS: I haven't played it but this looks pretty terrifying.

If you'd played it you realize how easy the game is and never be scared of it again xD

OT: I truthfully have never been scared of a videogame character before :p

From what I've played, either the Poison Headcrabs or the Stalker Necromorph from Dead Space 2
