Fallout 3 had many funny bugs. I killed a ghoul which proceeded to have what appeared to be an epileptic fit on the floor.

Left 4 Dead, second to last part of No Mercy.

So my friend was Zoey and I was Bill. The tank spawns as we're getting into the very construction heavy area so the walls are gone. I'm the first to be attacked and I'm sent flying into a wall and can only watch as my friend is shooting wildly at the tank and backing up into what I thought would be a wall so I was just screaming into my mic for him to run away or do something....But it wasn't a wall at all, it was the edge. I almost wet myself when I saw the tank just run right off the edge and francis ran over and lifted zoey back onto the floor....And it was controlled by a player too....

Gears of Wars 2 was chock full of this.

Giant Worms, overly done testosterone, when the wife is shot because she is no longer attractive...

This, but when you go through the area at night (before the fight).

I was stumbling around blindly waiting for my mousetrap to catch a Tsuchinoko.

I got bored of the dark and put my Night Vision on, just as a snake comes close to me.

Due to instict (I hate the poisonous snakes) I rolled out of the way, tripping a trap as I did so. A spiky log is swung down into my face, knocking me straight into the mud. XD

Halo 2, ended up flying a ghost upside down in the air in multiplayer, ran into a tank which flipped the ghost so high up in the air then somehow landed back up through the ground. Very weird but funny.

When Dom shot Maria, because it was just so out of character and seemed like such a bizarre scene compared to the rest of the game. MAARRRIIIAAA!!!!!! lol

This will make me look like a bad person but in Bad Company when you hit a soldier with the air strike meant for tanks and they go flying at high speeds towards walls and such.

Also pretty much anytime anything gets blown up so severely that their head get stuck in the ceiling and they are just hanging there

I laughed...although it was meant to be taken seriously. And oddly enough, wanted to listen to Rammstein's "Du Hast" afterward. Real bad.

Project Sylpheed. The intro to the first mission has you, your love interest and another pilot, Brandon, in a mock-dogfight with the squadron leader. When the leader gets behind Brandon, he quite literally freaks out, even though its only a training flight.

This, and the rather wimpy way he behaves after the dogfight and when your asked to investigate some unidentified fighters made me mentally tag the guy as 'dead meat'. Sure enough, the guy doesn't even make it out of the first cutscene. All through the mission there's pilot chatter of 'oh no, why did it have to be brandon?' as if its supposed to make us care that this sacrificial lamb character died... every time I heard one I had to fight the urge to grin at the narminess of it.

The funeral that follows is largely nicely done... just about everybody there is acting professionally... except your love interest, who's sobbing like a small child, even though she's in the military as a fighter pilot!

Which nicely leads into the next mission where the enemy launch an all-out assault on an allied planet... and amongst other gems she comes out with 'Oh no - this feels like a real war.'...

Again... shes in the freaking military as a freaking fighter pilot, and she can't seem to understand she's now on the bloody frontline in an ACTUAL SHOOTING WAR where people will die. What was she expecting when she joined the army?

I can give a lot of examples of people dying pointlessly just because the story required someone to die to mark the severity of a situation. Most of the time they die because they are just stupid, not because they were outmaneuvered like they should.
