It is the best game I've ever played (the only reason I play Dead Space is because sometimes, if I'm drunk enough, I can fool myself into thinking they remade SS2), but there's a difference between underrated and underplayed. Anybody who's actually played it puts it in the top five.

Arcanum is underplayed. Every reviewer talked about how it had the most iterations of character creation they'd ever seen (beat it 7 times-each with a completely different character) and a unique world, but nobody bought it.

Alpha Protocol was underrated. I see alot of it in Deus Ex:HR, especially the talking duels. And misplaced boss fights. It just wasn't as flashy and had more bugs. I can forgive the bugs because none of them broke the game for me, and I beat it 4 times. But the end level was kinda...retarded. Which stood out in such an otherwise intelligent game.

Vampire Masquerade: Bloodlines was massively underrated. Story and gameplay were great, it had a somehow fresh idea, and the graphics were great for their time. It just had some bugs so people didn't play it, but if that game had gotten a console release, Troika would still be around.

An open-world, sci-fi, third person shooter, rpg doesn't sound that original these days, with Mass Effect 3 around the corner, but in 1999 it was amazing.

I don't know about underrated but underplayed is Ratchet and Clank and Sly cooper. They both got kind of overshadowed by the Jak series but I liked them more.

Alan Wake was an amazing game! It really should have sold better - that's the only "underrated" game I've ever bought.

Brutal Legend (PS3/Xbox)
Resonance of Fate (PS3/Xbox)
Lock's Quest (DS)
3D .Dot game Heroes (PS3) Think Zelda 1 but with legos.
Rayman: Origins (PS3/Xbox) Extremely over-shadowed by Skyrim and CoD. One of the best platformers ever created.
Dark Cloud 2 (PS2)
Metal Arms: Glitch in the System (Gamecube)
Sonic Adventure 2: Battle (Gamecube) Before Sonic hit rock bottom.
Viewtiful Joe (Gamecube/PS2)
Psychonauts (PS2/Xbox)
Grim Fandango (PC)
Banko & Kazooie: Nuts n' Bolts (Xbox) ...I liked it.
A Space Shooter For Two Bucks! (PS3/PSP) If you have $2 bucks and a ps3 or psp, get this game! It's hilarious and can be quite challenging.

I like all the Sam and Max games, from Hit the Road to the last S&M (tee hee) chapter from Telltale. But I'd be hard pressed to rank any of them with those games. That's just me though.

I forgot another (genuinely) underrated game: Singularity. It's not going to set the world on fire, so to speak, but it's a genuinely fun, well-made nuclear apocalypse shooter that Lackovision kind of threw to the wolves. I'd recommend it to any FPS fan without hesitation.

Have to add some to my previous list:

Kurushi Final
Muppet Racemania (aside from being an awesome kart racer, it had the best transition of the Muppet Show opening song. Ever.)
Colin McRae Rally. The 1st one.
Dave Mirra Freestyle BMX Maximum Remix (the rag doll physics make me wanna play this just to crash in mid-air)
Soul Blade (before boobs took over, this and Soul Calibur were some of the best fighters out there)

Also, Pure and Split Second. Black Box had impressive new mechanics nailed on these 2 games and they were a lot of fun to play. I even named my ATVs.

In my experience these are some underrated games that I found to be quite enjoyable:

Enter The Matrix (PS2).
Fight Club (PS2). It received terrible reviews, but I found the fighting mechancis to be visceral and worth the very small price I paid for it (keep in mind it's been quit a long time since I've played it though).
Duke Nukem: Forever (PC). It wasn't THAT bad, well, for $17 AUD it wasn't bad.
Two Worlds (PC).
Hoard (PC). An enjoyable way to kill 15 minutes every so often.

I should of gone into more depth, but oh well.
