Bioshock. The part where you're in the room about to kill a surgeon and suddenly there's thick fog and he's gone. I expected what was going to happen next but fuck I was so shocked when the fog reappeared and he was directly in front of me. I think I wasted an entire magazine on that one.
Also, the same thing in the morgue with the shadow that's suddenly gone when you're round the corner but on your way back you get jumped from the dead people storage thing (what's the name again? Damnit).

OT: Well, Metro 2033 has given me a few small gasp moments. I haven't reached the infamous "library" level yet, so maybe my pants will meet their messing there.

I know that exact feeling. I had been exploring an abandoned mineshaft and kept being ambushed around a certain point even though I had fully lit the area. I later found out that the shaft cut through a cavern and there was a hole in the top of the shaft they were dropping down from. Seems silly of me to have not noticed it in the first place. But when I first didn't know why I was being attacked...

The beginning of the third encounter with Scarecrow in Batman: Arkham Asylum was less pant shitting and more, "Oh god what the fuck is- WHAT THE FUUUUUCK?!?" (Anyone who has played it knows what I'm talking about.)

This. When I first played, I turned the game right the hell off when I saw that. I couldn't find a picture of it, but when the light is off, he freaking glows red in the dark.

But FEAR... Any scene involving the dark and Alma and pants got ruined...

Along with the water part of Amnesia...

Condemned, the part with the mannequins. Jesus that was creepy. Also, the second Condemned where the Butcher's body starts moving (because I killed that fucker in the first game) and then you realize it is a body underneath him that is moving.

The first time I was being chased in the System Shock 2. The enemies were genuinely terrifying since they could pretty much decimate you in a few hits, so being chased by even two was terrifying. Then I fell off of the walkway I was on, took massive damage from the fall, and had no idea where the enemies chasing me were, arguably worse than knowing they were behind me.

The second time you meet Pyramid Head in the hospital made me nervous for the rest of Silent Hill 2. However, Dark Corners of the Earth had some scary moments.

The broken neck ghost in the first Fatal Frame.

You first see her as your wandering around, at just the edge you see a white figure fall to the ground and the detector turns warm(an indication of a hostile ghost).

You pull out the camera and look around. A kimono comes floating up to you... It's a little eerie but not that scary.

Then when she gets close she snaps around. You see her head bent back so that it is resting on her back, with bumps showing where her neck was broken. Her eyes and mouth are wide open and you hear the most creepy wail of all time.

First time I saw her (in a group playing with someone else playing) I got so scared I tried running. Only I had forgotten I was sitting in a chair. Needless to say a nasty bump on the head and plenty hugs of reassurance followed. XD

I had actually heard a tiny bit about it before getting there, so I was anticipating this thing the whole time.
The moment the level loaded I felt the atmosphere of unease. Nothing was alive, nothing moved. No lights, no guards.
It's a void.

Once I finally made my way inside to the inner area, and I walked up to the desk and saw the figure that darts past so quickly that you will have had wondered if you had seen anything at all. I fucking FROZE.

No me in real life, I sat in my lawn chair with my ice tea next to me, storm raging outside (yeah) and stared at the screen for 3 minutes.
I felt my mind actively telling me not to go any further. I felt true, unbelievable horror that night. I screamed, a lot. And I've never felt so much like a cowering mouse fleeing for my life.
I remember when I finished the level I felt a cauldron of emotions but mainly joy, relief and anxiety. My body was shaky for around 10 minutes after I turned the game off.
That was pretty much my exact Shalebridge Cradle experience, right down to the 3 minute freeze at the exact same moment as you while I processed what I thought I'd seen (whatwasthatwhatwasthatwhatwasthat), decided the game was still just jerking my chain, and proceeded.

It wasn't.

I must have had this happen to me at least five times today alone. Heck, any time something or someone comes up behind me and kills/nearly kills me when I'm a bit on edge always makes me jump.
