Spiders - can't be in the same room as one, too scared to get within distance where I could kill them, have to flee and call for backup.
Heights - can't get near the edge of a cliff/building etc without freaking out. Plus it really worries me when other people are close to the edge.
Mirrors - at night, I always avoid looking in mirrors, because I'm generally pretty scared that I'm gonna see something creepy in the mirror and then turn around and it's not gonna be there and then my life will become a shitty low-budget horror film.
