I've named heroes "HERO". I've named them after myself. I've named them after whatever I was thinking. I've named them the default. Basically, I just pick a random name and use it.

I usually reference to someone in my life, or a celeb, primarily in mass effect. all the other games that let you name them are, like, zelda games, where i tend to name him Link, but i've done fagballs, amongst other things 0.o

Referential. I'll either use an inside joke that I share with friends or have a theme I name all my characters. Once in a play through of FFVII I named all the characters after soda brands. Other than that, I'll go with default names.

I usually try to think of names relevant to the atmosphere (modern, medieval, etc.) although if the main character is really cool, on rare occasions I'll use my own name instead.

I have a pre made list of names I use which essentially reference eachother, but they fit certain types. For example, if I use Renald De'Mere, its likely a Paladin type. It was my NWN Paladin character, and my Knights of the Nine character.
But as my repitoir of characters increases, so do the names. Playing DnD it comes up more. I think a good deal about my character names though, but often I use names I like from other sources. I used the names Taven and Daigon for two important characters of my last campaign who were opposing brothers, named after the two opposing brothers from Mortal Kombat: Armaggeddon for example.

Console games I tend to just use the default name. If there is no name available, "Chaos" is my typical standby.

In the case of online games, Chaos gets taken within the first five minutes of a game going live, so I tend to be a little more unique. If the game has a considerable amount of lore behind it, and I know at least some of said lore beforehand, I'll try to make a name that's at least semi-lore-friendly.

Otherwise, I'll simply use referential; I'm particularly fond of the Japanese language, even though I can barely speak three sentences worth, so whenever I make a new character or name a new pet I'll pop up a Japanese Dictionary and look up words for a good half hour or so.

I've been chadachada or chadachada123 for basically as long as I've lived. In games with unique usernames (MMOs), I always go with one of those two.

Otherwise, I tend to use my name. That, or Chada, since that works too.

I suppose I use a mixture. I have a few standbys, Sariteiya and Aeldas being among them that I use when I don't particularly care. If I'm playing something where there's an established naming lore I usually try to fit in with it. With some games I go default, (Zelda, Final Fantasy etc). Sometimes I just try to make up a name, (As with Shepard), but then I usually end up staring at the naming screen for half an hour trying to make up my mind.

As an odd side note, when I make Sims of a different ethnicity, I research names to try to name them appropriately (Authentic Chinese, Indian, Middle Eastern names etc.) For some reason I'm afraid someone will call me on it if I use an Inauthentic name. Like there's some sort of Sims naming police.

I always name my characters after characters in the Shakespearian play Othello, and when given dialogue choices I try to answer as I think the character would have.
Dunno why, its just really fun.

I name them after characters from another game, except for Final Fantasy VIII. Squall is known as EMO GIT.

For tabletops, I usually use http://www.behindthename.com/random/ and filter based on the setting of the campaign.

For video games? I really don't know--usually it's just something that sounds nice to me. Kyrena is one that I often use for my female characters; it was the name of my first assassin in D&D, and it just stuck with me. I don't play many games that have customizable names, though. :S
