I usually prefer realistic gore with a couple of extra blood spurts or limbs flying everywhere because it adds satisfaction. i also think it makes games a lot more immersive if you shoot a guy and you see his arm fly off right in front of you, as oppose to him just falling down.
in games where you can cripple an opponents limb, eg. fallout, i like to have blood spurts to show that ive done so, but if the hit has no lasting effect on the enemy, then theres no point to have ridiculous amounts of gore.
