Pyramid Head from "Silent Hill 2". My heart always jumped into my throat.

Also, the Phase 1 Dark Troopers in the original "DARK FORCES"..

On the ice level... Right at the end of the level.. You're in the ventilation shaft in the
dark.. You're scrambling to find your way out.. Then you hear that sound that those Terminator
type Dark Troopers made...

And I realized.. IT WAS IN THE SHAFT WITH ME..SOMEWHERE...IN THE DARK.....

That's about the most scared i've ever been in a game.

Bloodsuckers in dark tunnels or at night. Snorks are a close second. S.T.A.L.K.E.R can scare the shit out of you at times.

This. Bloodsuckers were scary as hell sometimes. Not when you could see them out in the open but when you'd go exploring looking for new guns or ammo you'd climb up a set of stairs then when you went back down there'd be a set of glowing yellow eyes staring at you from the bottom. I got chased up a windmill by three of them once. Scared the hell out of me.

Loads. I remeber borrowing Starcraft from a friend and got scared by something and it ended up with me uninstalling it. I had something to do with the Zerg I think - I can't think for the life of me what it was about though lol.

One funny story though -
Playing the original FEAR with my little brother hiding behind the chair watching, in the dark. I had completed it, and was expecting alma to appear at the top of a ladder close to where I was. I went down this one ladder convinced it was at that point, and sure enough she appeared and I completely shat myself and moved my chair right back, knocing my brother down. That was fun :D

Single Player: anything from Condemned 1

Multi Player: the mouth breathing 14 year old american who screams louder than the headset can really handle whenever someone has the temerity to be marginally better than him.

Regenerators will always have a place in my heart as one of the best scariest enemies simply because of just the presence they have even before you see them. More often than not, they are hiding behind a wall, waiting and all you can hear is their breathing which somewhere between cat hocking up a ahirball and a drowning man. Then once it realizes you're nearby, the music, which is integral to the scare factor, kicks in. Ohhh, but it's not done yet good sir! The more you try to kill it, the more horrible it becomes until you finally luck out and kill the plagas...spores?... and it finally explodes like an overripe whale.

HL2 fast zombies also get an honorable nod, but they are lessened by the fact that after a while, I got used to their hollering and their glaring bulbous weakspot. Just let them get close, have shotgun, will travel.

Stranded 2 Lions, when you are as helpless as you are in this games beginning, nothing could be scarier.

Air Elementals in Nethack. Demogorgon is a close second due to the illness strike.
Cockatrices don't scare me much anymore. They're slow, and easy to dispose of as long as you do not let yourself become overwhelmed.

But really, the bloody Air Elementals: Either come equipped with wands of teleporation (or blessed scrolls of taming), get really lucky and avoid them, or lose your Ascension Run. Your choice.
