The Shadow Temple in The Legend of Zelda: Ocarina of Time. Those messages you read when you enter some of the doors, coupled with the music and scenery... it's just unnerving.

the intro to bioshock, not the awesome intro speech, or the plane crash, im talking about when
the spider splicer guts someone in front of your face before the bathysphere opens and then proceeds to try to cut her way in before disappearing. i was literally so scared (partially because this was at midnight or so), that i could not bring myself to exit the bathysphere. i had to try again the next day.

and some might say that it is not that scary, but remember, that at this point, you are completely unarmed, and the game doesn't let you use your fists. and she has frigging twin scythes!

and yes i know the thread is old, but i still like these threads.

The haunted mansion in VTMB Still scares the shit out of me, having to go through it again is one of the only reasons I dislike additional playthroughs.

In Skyrim, you hear a wolf noise, keep an eye out, check your map, adjust your heading, "oh shit where the fuck did you come from!"

Also Endermen from Minecraft, once I spent an entire night staring one down hoping nothing snuck up on me.

The haunted mansion in VTMB is very nerve racking, and kinda scary, especially the basement part.

First time I EVER played Fallout 3, I went to the Super Duper Mart and there was a fucking Deathclaw. I had no idea what to do, and it just cabooza'd me.

ah yes i remember early fallout 3 as well. i knew nothing of the franchise at the time, so that super duper mart unnerved me somethin' fierce. and then when a raider takes a potshot at you from somewhere you didn't see, too far away for your meagre perception to warn you of, thats a great unintentional jump-scare right there. the settings for fallout 3 were just fantastic.

I was going to get Far Cry 3 until I saw the trailer that showed there were sharks in the water.

Halo CE, first encounter with the flood. At first I was like "gonna kill some covenant", but then I was like "Jesus Christ what the fuck is that"
Really? I found the Flood in CE to be decidedly unthreatening (using the assault rifle to shoot their arms off and watch them chase you around effectively naked was hilarious), but in Halo 2 they were absolutely fucking vicious. I loved it. Fuck that dual-wielding shit, though.

Dead Space 2 (which I consider an action game, seriously). Cross my heart and hope to die...

Urban Chaos: Riot Response. That very last level, where you start out without a weapon or riot shield? Those of you who've played it know what I'm talking about.
