Ehh...the ending was alright...much like the rest of the story. Don't get me wrong, the game is great, but the actual plot is a bit by-the-numbers, and the ending was basically the INCREDIBLY SINISTER OUTSIDER WHO'S MOTIVATIONS WERE NEVER REVEALED saying "Yeah, everything's great. Plague's cured. No, we're not going to tell you how."
Since when was the outsider incredibly sinister? He just enjoys watching the events play out, maybe poking things in some ways along the way. If you're using nonlethal methods, he'll say that he loves how you hold back and listed when others would shout out in rage. Rather than good or evil, I think he's neutral.
As for the plague, it's only natural that it dies out eventually. Even by the end of the game, if you're playing low chaos, people are saying that there's fewer rats around, hinting that the spread is already slowing.

I just finished it as well, pretty damn good. Got the low chaos ending because that's how I roll.

The Outsider reminds me of me. I've always been more of an observer rather than a do-er. 'S just more interesting that way.

As for how the plague stopped... There're two known and proven working preventatives on the market. That's sort of all you need to use when you finally get your shit together Mr Government.

There really are all sorts of ways to complete missions, it's actually pretty amazing the level of detail and work they've obviously put into this.

Oh and The Church of this game is doing my natural distaste of religion in video games no favours. Dicks! Dicks everywhere!

Oh, I don't know, maybe the fact that ity's heavily implied that the Heart is the Empress'? Or maybe that the league of assassins that killed the Empress all have the Outsider's superpowers? That doesn't set off the alarm bells for you?

Just speculation on my part, but I do believe that the reasoning for using the Empress' heart to create the Heart for Corvo was a subtle allusion to what the Outsider implies in the end "but you were MORE than just the Royal Protector [with the Empress}, weren't you". That Corvo held the heart of the Empress while she was alive, and now her heart (soul) guides him in death to avenge her murder by hinting to the locations of powerful occult objects that will further empower his Outsider abilities to do so. As well, saving her daughter
and from what the Outsider not-so-subtly implied, Corvo's daughter too
and heir from those whom would use her for their own deplorable gains.

As for the Daud's Assassin's having the Outsider's powers as well, that is rather simple. The Outsider gives individuals whom he deems interesting the powers in order to "test" them, I think. To observe how humanity comes to terms with these supernatural forces being used in the mortal world, and how the users of the power influence the world through their new-found power (in Corvo's case, how he uses his powers to change Dunwall's fate and future, through bloodshed and chaos or through shadows and non-lethal approaches). It doesn't really matter to the Outsider how the individual users their powers for either good or for evil against others, as made known when he states to Corvo "It is up to you how you choose to use these gifts [I but observe]."

The Assassin's choosing to murder the Empress for the soon-Lord Regent was nor favored nor disfavored by the Outsider. The Outsider isn't omniscient (I don't think so?).

I don't think he is, he expresses surprise and interest in many of your decisions throughout the game.

OT: I got the low chaos ending, gonna get straight back in and do another run!
