Day of Defeat: Source onwards any weapon can potentially kill you on the first hit. From SWAT 3 onwards you almost certainly will be killed on the first hit. Whilst with SWAT4/Red Orchestra/Flashpoint if you don't have a detailed plan with multiple contingencies BEFORE you enter a mission you may as well forget it.

Depends on how hardcore you want yor simulation.

If you have Half-Life 2, I recommend you download the multiplayer mod Insurgency. It's pretty authentic, you don't have a hud and if you get shot you die. No regenerating health or med-packs. It's damn good with a decent team, and it's free too!.

Arma 2? Apparently its not a game its a battle sim. Haven't played it yet though.

From everything I've heard ARMA2 is the way to go. I played the demo but had a hard time getting into it.

Stalker is reasonably realistic but not terribly tactical, but definitely a great game (the LURK mod comes highly recommended if you get it).

SWAT 4 is great fun with friends, pretty tactical as well

If you have Half-Life 2 and want a cheap way to get a single-player tactical experience, you could download SMOD Tactical, which gives you a bunch of real-world weapons, throws in iron-sight aiming and seriously tightens up the difficulty (one shot to the head WILL kill you). It's kind of fun traipsing around City 17 with an FN P90 equipped with a reflex sight.

I loved Republic Commando! But not what I'm looking for, at all. In fact, 180 degrees in the other direction.
Yeah, it's far from realistic but it's still fun.

Also you could try STALKER. I've never played it but people generally agree it's quite good (not too sure about the realism).
It's more realistic then most shooters. Mainly because you die faster then a dream. You know what happens when you don't take cover? Well you die. It's a great game, but there's like only three types of food, three status changing items, and no explanation for where nearly all of them come from. Where the heck do they get bread in the Zone and how do they survive when they only drink Vodka?

I highly recommend Project Reality: It's a total conversion mod for Battlefield 2 with the intention of replicating reality. BF2 itself is getting on now so the graphics aren't up to much, but the gameplay more than makes up for it.

If it doesn't disgust you to much go French, look at a lot of Ubi Soft releases like Rainbow 6, Ghost Recon... well anything that says Tom Clancy on the cover really. If you want Sneaking look into Splinter Cell. Or if you are really crazy send those Eastern-Europeans some money and look into Operation Flashpoint, AmrA2 or the S.T.A.L.K.E.R series.

(I'm not trying to offend you French or eastern-Europeans, I'm just being a dick)

To think Activision/IW wants to eliminated any chance of something this beautiful will ever happen again...

If it goes through the brain or the heart it's realistic. But considering in most games your character is wearing at least something like kevlar, you'd be alright most of the time.

With a cousin police officer and several friends in the marines, I have confidence when I say that's BS. These days combat-competent weapons have specialized munitions to destroy, of all things, kevlar. The only 100% way to be sure you will not die from being shot is to utilize cover.

First of all: Your cousin is a ******. Sorry.
Secondly: I'm well aware of this, but it helps anyway, and most enemies you encounter in video games are not using these kinds of weapons.

I play lots o' war games, and these days the AK47 standard munitions have the basic necessities for piercing kevlar (otherwise US troops would be invincible against half the world's armies).
