First thing?
Well, before I actually PLAY the game, I read the back of the box, turn down the lights, grab a blanket and pillow, make sure absolutely nothing can distract me, grab a soda, and play.
That or check the controls in the options menu.

1st: Remove the manual from the disc case. I don't know why, since I never seem to look at it these days. Possibly goes back to the days where I'd open a new game in the car since someone else was driving so I had time on my hands to think about the game.
2nd: Scan the back of the game case. Once again, it's just a habit that I probably picked up from car rides home with a new game.
3rd: Don't press a single button and watch the intro scene.
4th: Start on easy if it's the story I want first (only did this with Prototype because I wanted to have all the fun I could with the already overpowered Mercer). Normal on other times.
5th: Give a slight test of the controls before testing how fast I'm allowed to move. If it's safe when I start, I'll also take a good look around.
6th: Evaluate voice acting once the 10th cutscene comes around. It's around this time that I'll turn subtitles on and either continue on or turn the volume down a bit more.
7th: Ask myself if I'm having fun
8th: Miss a meal by accident.
9th: Curse the game for that
10th: Save

From then on, everything else is just natural.

I usually check the achievements if there are any to make sure I pick them up so I don't have to play through again just to pick up one or two things I missed.

In ME2 the first thing I did was whip out a sniper rifle and check if the 'drunk guy with a rifle' aiming mechanic was still there (it's not)
Well you couldn't because you don't get a weapon until you get resurrected.

I first change my resolution if it isn't 1920x1080 already, then I change every setting to max.
I look at the control scheme and try to bind everything to the closest thing to my normal one.
Code:
unbindall
bind "0" "slot10"
bind "1" "slot1"
bind "2" "slot2"
bind "3" "slot3"
bind "4" "slot4"
bind "5" "slot5"
bind "6" "slot6"
bind "7" "slot7"
bind "8" "slot8"
bind "9" "slot9"
bind "a" "+moveleft"
bind "b" "lastdisguise/lastsecondary"
bind "c" "+voicerecord"
bind "d" "+moveright"
bind "e" "voicemenu 0 0 (MEDIC!)"
bind "i" "showmapinfo"
bind "j" "dropitem"
bind "m" "changeteam"
bind "n" "changeclass"
bind "o" "+quickswitch/weapon inventory"
bind "q" "lastinv"
bind "r" "+reload"
bind "s" "+back"
bind "t" "impulse 201/tag/spray"
bind "u" "say_team (team chat)"
bind "v" "voice_menu_3"
bind "w" "+forward"
bind "x" "voice_menu_2"
bind "y" "say (chat)"
bind "z" "voice_menu_1"
bind "KP_PGDN" "say I regret nothing; explode"
bind "KP_HOME" "say_team pyro burn me so i get flaming arrows/key saying 1"
bind "KP_UPARROW" "say_team SPY BEHIND YOU!!/key saying 2"
bind "[" "cl_drawhud 0"
bind "]" "cl_drawhud 1"
bind "'" "+moveup"
bind "`" "toggleconsole"
bind "/" "+movedown"
bind "\" "voicemenu 2 2"
bind "-" "admin"
bind "=" "screenshot"
bind "SPACE" "+jump"
bind "TAB" "+showscores"
bind "ESCAPE" "cancelselect"
bind "INS" "+klook"
bind "DEL" "playaudio"
bind "END" "centerview"
bind "PGUP" "+lookup"
bind "PGDN" "+lookdown"
bind "PAUSE" "pause"
bind "ALT" "taunt"
bind "CTRL" "+duck"
bind "F1" "+showroundinfo"
bind "F3" "askconnect_accept"
bind "F10" "quit prompt"
bind "MOUSE1" "+attack"
bind "MOUSE2" "+attack2"
bind "MOUSE4" "blast/strange fire/glitch attack"
bind "MOUSE5" "inspect (item/player)"
bind "MWHEELUP" "+jump"
bind "MWHEELDOWN" "+jump"

And first thing I do when I enter the game will be change my mouse sensitivity to the closest to my natural setting.

I usually don't do anything super crazy or weird. Just watch the cutscene and scope out the main menu, then once I start the actual game I check out the start menu

Read the manual for a console game
Bring up the various menu screens and look for clues on collectables in game
Look at the map
Attempt to use each of the main functions (run forward, run sideways, shoot, switch weapons, jump, etc.)
Follow the helpful on screen narratives (unless the game suddenly throws me in a huge boss fight like dick games do).

I tweak all the setting to unnecessary fineness, most of the tweaks are unneeded but it makes me feel important.

also, i customize the controls if it lets me

Absolutely nothing out of the ordinary.
I work out the controls and basic game mechanics, change anything I feel needs changing, then play the damn game.

Oh, and I always keep an eye out on how easy it is to exploit certain features of a game just for fun (For example borderlands and being able to flip the car over walls with ridiculous ease to get out of the level)

Go Klepto, try to see what degree of item interaction it has (To assist in the klepto) and spam any starter special moves in my first fight.

Mostly Kleptomaniacy though.

1.Watch the intro & cut-scenes religiously.
2.Play a half an hour with the "character creation" settings.
3.Star game play for about an hour or so and realise that my character's fucking ugly.
4.Restart game, make new character, get bored during cut-scenes.
5.Continue playing to the end.

Turn the subtitles on. I don't know why, but if a game has a choice to have subtitles on, I'm going with that choice.
