I was actually thinking the other day how a Puzzle RPG could be good.

If the combat played out in some sort of bejeweled like puzzle

You have a group of characters, like any JRPG, you choose attack, and instead of some quicktime event dealy where you hit the R button or whatever, a small 4/4 bejeweled type game pops up, and you beat that as quickly as possible.

...

This is why I don't design games for realz.

THE game has already been done in 1991: Civilization

I quote:
This game just screams "best game in the entire history of the universe"!

having recently watched Dead Set and replayed Resi Evil 1 on the Cube, my mate and I were discussing the ultimate zombie game. Basically it would be like a cross between Fallout 3 (with different factions to associate yourself with, and rpg elements involving combat and leadership of people) Dead Rising (with the idea that everything is a weapon, and that you can lead survivors to a safe house) 28 Days Later (in that they wouldn't be zombies, but the fast running infected, and a lot of the time, your best option is to just run away- making it more survival based than combat, unless there is only one, in which case the combat could be similar to Condemned 2) GTA IV (in that you can borrow any vehicle, and attempt to use weapons you find, and also controls and feel of the game as you play would be very similar to this) and San Andreas (you start off in a rural environment, like that bit that was in the bottom left corner near the mountain). However, the main emphasis of the game would be survival, so you have to find food for yourself and the rest of your fellows who you may have saved. All people would have a purpose, like if you rescue a soldier, he'll use combat to keep people away from you. Save a doctor, he'll heal any wounds you might get. Or you can say screw you to them and run for it. In this way, the squad element would be kind of Bioware-ish (think Mass Effect or KOTOR). The story would have to require you to keep moving around, so that you go from small village to medium town to full scale war and sheltering in a city.

I'd call it "28 Condemned Days after the Rising Fallout: Mass Theft Andreas Republic"

You basically have two choices:
Fun; i.e. basically Pong.
Hyper-realistic; i.e. the Matrix.
matrix pong!
the most advanced AI and grafix settings!
you actually feel like your the pong block!
You need to play Plasma Pong. It's pretty epic.

Unfortunately. -THE- Ultimate Game, would probably kill gaming altogether, either, it would be so good that it would end up all anyone wants to play, or, so good that, you can't imagine playing any games ever again, after all, you've played the ultimate game, what's left that can compare?
Or you would never stop playing as it would consume you and show you how much better it is than life (See Red Dwarf - Better than Life). The game evolves from the impulses in the player's subconscious - giving them the life they always wanted. Their bodies started to wither away and die, and they had no idea it was happening.

For these reasons, I'm glad we haven't made the ultimate game yet. Besides, if there were no more shit games, what would ahtzee have to complain about ;)

An impossible feat, as detailed by above posters. But if I was honestly challenged at attempting such a task, none of my own personal designs would qualify... We'd want something with complete customizability in terms of battle mode, camera type, pacing and class type.
For example, if you want a turn-based overhead combat mode, or first-person style, you set it in a menu at the start of your file. Hire a few professional book writers for an extremely flexible fantasy-type story that branches and changes depending on how evil/heroic you act (Bernard Cornwell pops to the top of my list- he's very good at the grim and gruesome aspects of life before running water; get Aaron Allston to write the funny bits). Get the team of artists that created Prince of Persia and World of Warcraft's art together for 3 years to create the environments and characters. Finally, include a 'New Game +' feature and allow friends to control allied characters online if the main player wishes it.

Or to put it in a less elaborate/expensive way, just fuse the first three Secret of Mana/Seiken Densetsu games together, give them a longer, coherent story and pretty them up using current-gem graphics and music, and maybe tone down on the top annoyances of JRPGs featured in it ('Chosen One', obscure class-changes, etc.).
