Freelancer (PC) - Really well designed Space-Sim.
Dark Messiah (PC)- Seems to be like it's a bit of a "Vegemite" game.
Fallout: Tactics (PC)

Secnd on Red Dead Revolver and Septerra Core.

As far as my own votes go: No More Heroes (Wii), Skyblazer (SNES), And, probably the most underrated game of all time, Zone of the Enders: The Second Runner.

PSI-Ops, and second sight. How the fuck do you not enjoy a Third Person game with the ability to use psychic abilities to fool around with and kill soldiers in funny, creative ways?

Lufia: The Legend Returns (and the entire Lufia series except for Ruins of Lore, I find that game to be the redheaded stepchild of the series), Robopon 2, Astro Boy: The Omega Factor, Dragon Quest 5, and the Lunar series.

I'll throw in a few gems I remember back in the old days.

Vandal Hearts - 1 and 2 were my first tastes of Strategy RPGs. Okay story, but a wonderful difficulty. Tactics were heavily needed in these games. Konami needs to bring this one back.

Saiyuki - Another Strategy RPG, this one based off of the story Journey to the West. For an older game, this RPG had a ton of extras you could do. Great job by Koei on this one.

Ring of Red - Yeah, another SRPG... this one's set in an alternate reality Japan WWII type setting, with diesel feuled mechs you walked around in firing mortars and machine guns.

R.A.D. - Robot Alchemic Drive. A bad japanese giant mech B movie turned into a game. You vs. giant aliens in a fully destructable city. Tons of missions, goofy plot and dialogue, a unique battle system where you're piloting the mech from the outside, and can only battle from the character's POV. Never has there been a game so awful and so awesome at the same time.

Thousand Arms - RPG and date simulator in one... probably the only one that has and will ever come to the States. Dating was hokey, but it did have a purpose, and the story wasn't that bad. If you can find this one, try it out.

Hooligans - A Soccer riot simulator for the PC. Honestly, it doesn't get more awesome than that.

Mafia - A great game about the Italian Mob that was dwarfed by GTA3 back in the day. I'm estatic that this game gets a sequel.

Awesome stuff, innovative cover system and one of the first to use such a system.
Wow, I had forgotton about this game. I loved it. Thought the cover system and the blind shooting around the corner was awesome. Especially the blind fire. Haha everyone should go play this game so they can ignore all future "cover system comparisons" that say Gears of War started it....

The Warriors (PS2)
Come the hell on, its by rockstar, how could this not have sold a bajillion copies, it was an epitome of awesomeness in the ps2 era but no ones freaking heard of it.
What a game! I'm a massive fan of the film and the game was almost as good, although I recently brought a PS2 (i had it on the old xbox) and its platinum, so I guess a few people did buy it!

Other than that being from the PS1 generation I'd have to say Final Fantasy VI is no where near talked about enough in circles.
Tenchu 2
Fighting Force was epic.
SNK vs Capcom 2
Robocop Vs Terminator

Sorry if these aren't "hidden gems" my youth renders me unsure if most of these were actual gems or not.

There's this old SNES title called "On The Ball," an action/puzzle game that I love.
