Other than that, probably reaching level 30 in Mass Effect 2 after starting my second playthrough.

Engineer to Eternity (As a soldier, kill an Engineer WHILE he's repairing his sentry WHILE the sentry is being attacked by a team-mate.)
War Crime and Punishment (As a Soldier, in a single life, kill 3 enemies who have damaged a Medic that is healing you.)
Spynal Tap (As a Demoman, kill 20 spies within 5 seconds of them sapping a friendly building.)

Before that it was getting all Single-Player achs in GTA IV.

That took me to 323/326 (Steam Friend ones still outstanding)
