Got my face flattened by a tire in Halo 2. seriously this thing must have been travelling at mock3 it made my body rag doll across the map.

I got booted from a game of Halo 3 once when i fired a rocket launcher at the same moment my teammate decided to place a bubble shield.

He could see that i was about to fire the damn thing into 4 guys..
I threw a grenade at one guy (I don't remember what game) but I killed him before it went off and I ran towards it without remembering about it. I'm sure I don't have to explain what happened next.
In CoD4 on Countdown, I threw a grenade at a guy, but he got away in time, I wasn't looking, and I heard a grenade explode. Thinking it was mine, I ran around the corner and died by my own grenade that hadn't exploded yet.

I was playing Modern Warfare 2, threw some semtax. At the exact moment i threw it, a teammate runs in front of me and it sticks to him. I try to move, but he was being shot at so he retreated with me. Killed us both.......he was mad.

My most recent fail moment involves Oblivion, my character and a gigantic mountain. There was a quest where I had to climb up the tallest mountain and slay the orc that was at the top. After 20 (real time) minutes, I make it to the top and I couldn't believe it was finally over. Cue the raging orc charging towards me from the fog and knocking me right off the mountain. I had forgotten to save at all. *facepalm*

OT: Rocket lancher death in Halo 3. Spun around to shoot a guy in CTF but right as I pulled the trigger, here come mr.flagholder who lands 2 inches from my face. We both died and lost the game.

valhalla big team mutiflag, got to the flag with a warthog full of blues, we all jump out, storm base, get flag. run back towards hog, red grenades us, all survive, hog upside down, blue with flag running toward me as I flip hog. We have enough time to happily get away. What happens? I flip the hog, and it spins and hits me in the head, suiciding me! I could of at least got a suicide splatter achievement. We didn't get that flag....
