Fps jumping puzzles. You can't see your feet. No.
Thanks to the fashion phenomenon called realism, that's slowly changing. (But it won't make jumping puzzles any better..)

I am enraged that I cannot kill children in video games. I mean they are the most annoying little buggers why cant I just take a shotgun to their heads?
This pisses me off to no end in Fallout 3. There is a mod I've been meaning to try out when I have access to my PC again. Let's just say it involves Little Lamplight and a the Ripper weapon.
Hmm never heard of it. Seems I'll have to check it out

When people use the same cheap tactics over and over and show no variation.
So just the ordinary run of the mill multiplayer/online game then?
Mostly FPS games, like Counter-Strike, HL2: Deathmatch, etc. I like when people can use more than 2 weapons effectively, for example.

Snipers.

I HATE snipers so much. I don't care that most FPS players like being snipers. Its the least skilled way to play any FPS. Try actually playing the game instead of hiding in a little hill keeping your mouse at the same spot where you know someones head will always be at regardless of how you enter.
Have you ever seen quick scoping with teh awp in css? Thats the tactic I use and it takes more skill to use that any other weapon in the game. yet people still say that the awp is a newb weapon that takes no skill. That is a complete LIE

Boss battles in Dead Rising caused me to destroy one of my x-box 360 controllers in a fit of rage recently.

Haha, you have no idea how proper sniping works. and also, they are playing the game, you see, the sniping weapons or sniping classes are in the game, and as such they are part of the game.

Exactly! In most games you cannot even make them bleed... now that I think about it you can't do that in any game.

When a game tells you who you can and cannot kill.

I DON'T care if I need that NPC to finish the game. I just want to KILL HIM WITH HIS OWN SKULL because he's so damn annoying! Often times it just gets worse when I'm supposed to defend the bastard. And if it's a child... Who in their right mind would think the player would listien to a kid?

Bugs. The simple reason for this is because I might enjoy World at War if were able to play the levels without waiting 5 mins for the information movie to come up.

Then, occasionally, Spies and Snipers!

clipping issues hurdle me out of the game. Immersion killer that instantly leads to me raging(especially if its at a high stress moment in the game).

Poor level design.
I hate it when I spend more time in a level looking for where I need to go or finding what I need to move ahead than I spend actually fighting or solving puzzles.
Case in point; any indoor level in Mirror's Edge.

Major game imbalances. Take Battlefield 2142 for example: The "Soldier" class is merged with the "Medic" class so that the same class gets both the most powerful anti-personell weapons and the ability to heal and bring other players back to life.
In infantry-only (I/O) maps/servers there's little reason to use any other class, so few players do. Despite this I still love the game though... it just could have been so much better.

I believe these "cheap tactics" generally stem from game imbalances, or just poor game design.
A number of players will only use the most powerful tactics/weapons/skill combinations etc. in the game, because; why should they use anything else if this option is better?
"Group Think" then comes into play, whereby each player assumes that the other players will be using said powerful weapons/tactics and therefore choose to use them themselves to avoid being at a disadvantage, resulting in almost everyone using it even if few of the players there actually want to do so.
