Just 5 minutes ago i finished a Left 4 Dead campaign in single player with the intent of getting the Safety First achievement. I played through the whole thing with no friendly fire and we never went back to a checkpoint (not sure if that matters or not.) Upon completing the campaign i watched the statistics screen and saw that neither me or any of the AI had any type of friendly fire incidents. I did not, however, receive the achievement. So my question is: Does anyone know why this happened and has this ever happened to any of you?

I had something similar, where I swear I didn't shoot a teammate but didn't get the achievement. (Played with bot's so they didn't get me either)

Tips I'd suggest (What I did to ensure I got it the 2nd time around):

1: Shoot as little as possible, melee instead most of the time.
2: Do not use any explosives of any kind.
3: Do it with bots rather than people, they won't ever hit you (never have to me anyway) and are good at killing thanks to auto-aim.

When it came to things such as the tank I only fired a few rounds when I had a completely clear shot.

I find the "no healing" one more annoying to be honest; I get 3/4 the way through a map then the bot's decide they like watching me get clawed to pieces by a hunter rather than helping. -_-

For the no healing you can pop the pills like they're M&M's. Also you can die and get incapacitated as many times as you want. For this i would recommend doing it with human players because the bots are programmed to heal you if you get below 50%, and they WONT stop trying to heal you, forcing you to stop every few feet.

It might be because you detonated a bomb or gas can near one of your teammates. Generally if they get hit by this it doesn't pop up because the damage is so negligible that it's not worth it but I'd think it still counts towards the achievement.

Actually there is an easy way around that (other than not get hurt :D ). Whenever the AI comes to heal you start healing yourself and they will stop. Just don't let your character finish.

Would meleeing a team mate count as friendly fire?

Nope, it doesn't. It would make saving people extremely hard if it did.

As already stated, meleeing is the way to go. There are two problems that I've noticed with gunfire and this achievement. First, you aren't always given the naughty message (don't shoot teammates) when you hit one of them, so if you're even close, you can't be sure that you did not hit them. Second, aiming over one of their shoulders will sometimes give you friendly fire even if you clearly do not hit them. I got it playing split-screen with a friend on easy.

I have completed every campaign on expert but it only gave me achievement for 2 thus far, and let me tell you that feels like valve personally kicked me in the balls. All i can say is, shit happens.

But i forgive them, because the game is awesome.

I didnt think it would, but i was just considering all my options because i would just spam the melee during the climax events and would usually hit the AI a few times.

Did you yourself finish the level or just your friends? You have to actually survive through the campaign, not just some of your friends. Back on the original topic, it is annoying that it doesn't always give you the "don't shoot teammates" message. My friend and I were playing and we each counted that we had shot a friend only once. When the credits rolled it showed that I had 7 friendly fires and he had 6. So it's difficult to know for sure if you will get the achievement.

The Ai director hates you...you may think I'm joking but I'm not. Its a real problem talk to much smack or mention how theres not to many tanks, or even talk to much about glitching and the Ai director will start denying achievements to you and causing tanks to respawn like crazy. A friend an I even put it to the test, so after around three or four hours of trying to get the untouchables achievement, we simply didn't talk for the entire round and the result was a resound Bleep bloop of achievement unlocked. I mean how many times have you said "I hope a tank doesnt come", or even thought "I hope a swarm of zombies doesnt randomly appear" only to hear the cries of the dreaded enemy a few seconds later.

Im dead serious, the AI director is something more then valve lets onto.
and Im paranoid.

Yup, am referring to times when i escaped.

And i hasted to remind everyone that the 'AI Director' is a personification of a spawn system, a very intelligent system it may be, but 'he' doesn't hate you, 'he' can't, because AI cannot hate xD

I heard that you have to sacrifice at least 27 virgins soaked in oil to get that ONE achievement that you need to finish the game... have you tested this?
